Oversees day to day project management of clinical outcomes improvement for assigned clinical areas, services or medical staff teams. Works collaboratively with leaders, clinicians and other staff in the development and implementation of strategies to identify and reduce risks, improve quality and safety and meet regulatory requirements.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
Identifies opportunities that impede attainment of clinical outcomes; analyzes and evaluates clinical data as it relates to clinical outcomes and resource utilization; interprets this information for other health care providers, communicating necessary strategies for improvement.Facilitates system level clinical programs with process improvement goals and strategies in collaboration with continuous improvement (CI); integrates standards of care/clinical guidelines/protocols in collaboration with multidisciplinary groups and evaluates for outcomes improvement.Serves as a clinical and systems resource; provides education regarding clinical data as it pertains to clinical outcomes, resource utilization and clinical efficiency. Presents educational programs or assists in the development of educational materials related to clinical areas of expertise/improvement efforts. Maintains awareness and knowledge of best practice strategies, disseminating information to relevant groups.Works collaboratively to develop appropriate scorecards/reports that drive continuous improvement and provide patient value.Develops annual outcomes report for designated clinical programs and provides a regular cadence of outcomes reporting to the clinical programs, ministry leadership, and other designated groups as requested. Maintains a close working relationship with leadership to optimize the quality and patient safety culture. Assists with ambulatory, post-acute and regional quality initiatives as applicable.Conducts leading practice research on clinical outcomes improvement and safety best practices. Participates in activities supportive of ongoing improvement of medical staff quality.Assists with evaluation, potential development and maintenance of databases necessary for optimum clinical outcomes improvement.Performs other duties as assignedEDUCATION
Bachelor's degree in Nursing from an accredited school of professional nursingEXPERIENCE

Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.Frequent keyboard use/data entry.Occasional bending, stooping, kneeling, squatting, twisting and gripping.Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.Rare climbing.Work Shift:
