Geriatric ServicesJob SummaryResponsible for providing evaluation and treatment upon physician referral to a patient population consisting of varying diagnosis Assess identify and treat all areas of deficits impacting leisure social interaction skills community resources and community re entry Provides instruction and education to the patientcaregiver to promote the safe execution of the facilitator techniques taughtJob ResponsibilitiesPlans organizes and coordinates recreation activities functional leisure skills and leisure education Interviews the person served to assess capabilitiesinterestsbarriers Monitor progress and recommends changes modifications to program as requiredPlan and implement interventions to prevent harm to a patientEngage and instruct patients in therapeutic activities such as games sports crafts music and group activitiesObserve analyze and record patients participation reactions and progress during treatment sessions modifying treatment programs as neededConsult with physicians and other members of the rehabilitation team regarding therapeutic recreation goals for program referrals Coordinate activities with staffrehabilitation team and participate in care planning meetingsAssess patients needs through observations medical records tests and talking with other healthcare professionals patientsrsquo families and patientsCreate treatment plans and programs that meet patientsrsquo rehabilitation needs and interestsConfer with members of treatment team to plan and evaluate therapy programsParticipate in discharge planning
