Referral Coordinator

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES (including, but not limited to): 

Prioritize referrals according to level of urgency, schedules patient appointments as necessary and communicates referral information to patients. 

All STAT and ASAP referrals are processed within one business day.  Routine referrals should be processed within 3 business days, routine screening consult within 5 business days. 

Follow up on any specialty/provider which has not sent summaries/reports to office. 

Ensure complete and accurate registration, including patient demographic, current insurance information. 

Assemble information concerning patient’s clinical background and referral needs.  Per referral guidelines, provide appropriate clinical information to specialty. 

Maintain referrals tracking system and up to date referrals appropriate documentation of referrals to promote team awareness. 

Patients are notified verbally and given/sent written confirmation regarding their appointment time, location, special instructions. 

Coordinate appointments as needed with outside facilities and specialist. 

Follow up on prior authorizations is performed at least weekly and notes made in the referral notes section. 

Follow up on pending referrals on a daily basis. 

Provides a standard of excellence as it relates to customer service and satisfaction for all customers. 

Communicates with all stakeholders, staff, providers, payers and patients as appropriate. 

Performs other duties as required.
