Position Summary As the Referral Coordinator, you will be the first point of contact for individuals and families seeking services. Your role will be pivotal in centralizing and managing the referral process, ensuring efficiency, accuracy, and an exceptional experience for those we serve. From collaborating with funding sources to maintaining compliance and generating actionable insights for leadership, your work will enhance operational excellence and foster program growth.
Key Responsibilities
· Serve as the First Point of Contact: Welcome and assist individuals, families, and referral sources, ensuring all inquiries are accurately logged into a centralized database.
· Coordinate and Distribute Referrals: Track and assign referrals across departments, ensuring timely responses and seamless transitions for individuals.
· Facilitate Application and Intake Processes: Assist individuals and families in completing applications and gathering required documentation, optimizing service matching and ensuring compliance with agency standards.
· Maintain Accurate Databases: Update and manage referral data to ensure records are precise, accessible, and support effective decision-making.
· Generate Reports: Provide leadership with actionable insights through regular reports on referral trends, timelines, and program utilization.
· Collaborate with Funding Sources: Partner with DDS, MRC, ABI, and other agencies to ensure compliance and streamline referral processes.
· Support Marketing Efforts: Work on initiatives to raise awareness of The Arc’s services and strengthen relationships with community stakeholder.
What You Bring
· Education: Bachelor’s degree in Psychology, Social Work, Human Services, Business Administration, or related field (preferred). Equivalent experience (2–3 years in referral coordination, intake, or case management) will be considered.
· Experience: Familiarity with human services, healthcare, or nonprofit environments and working with electronic databases such as iCentrix or similar platforms.
· Skills:
o Strong organizational and multitasking abilities to manage referrals and documentation efficiently.
o Excellent verbal and written communication skills to engage with individuals, families, and funding agencies.
o Proficiency in Microsoft Office Suite and comfort with CRM tools or electronic records systems.
o Ability to work collaboratively with diverse stakeholders and foster relationships with referral sources.
· Requirements:
o Valid driver’s license, acceptable CORI/SORI/Driving record.
o Certifications in CPR, First Aid, and CPI (or willingness to obtain).
