Position Purpose:

Referral Support Specialist- Contingent- weekend coverage.

The Customer Care Center (C3) Referral Support Specialist role exists to support the HCCs (hospital- based liaisons) with referral entry and referral insurance verification. The RSS person is responsible for accepting, importing and entering referrals from our RHMs into HCHB. This role is also responsible for the insurance authorization for certain payers.

What You Will Do:

Responsible for determining appropriateness of care, referral screening and acceptance based on THAH guidelines.Responsible for accurate documentation in the EMR and manages the referral from start to finish ensuring minimum data set is obtained and verified.Demonstrates high level computer skill so that the referral is transmitted in a timely manner to the appropriate agency contact for efficient scheduling.Meets and exceeds the set productivity standards for referral management completion and demonstrates ownership for follow-up needed for referral management to ensure patients are seen within established guidelines. Responsible for  Trinity Health Living our Values, Organization Integrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.Documents all standard referral elements into HCHB responsible for navigating multiple applications and online systems.              Acts as the central liaison between the agency and the referral source regarding the referral process, and serves as a primary contact/liaison with customers and families.  The duties and responsibilities described are not a comprehensive list and additional tasks may be assigned from time to time, as the scope of the job may change as necessitated by business demands

Minimum Qualifications:

Associate's degree preferred, High School diploma required with equivalent 2-4 years industry experience. Working knowledge of governmental reimbursement criteria.  Knowledge of third party / managed care contracts related to THAH services preferred.Microsoft computer skill set required. Experience with THAH current software platforms and applications preferred. Able to consistently demonstrate commitment to the mission and organizational code of ethics, and exhibits Behaviors of Excellence.Previous experience working in a healthcare environment, demonstrating computer literacy
