SCOPE AND PURPOSE OF POSITION:

The Care Coordinator Supervisor is responsible for the supervision of assigned staff and is accountable for adherence to regulations and policies of The Salvation Army, Lead Health Home and NYS DOH. Supervisor will support staff to integrate models of care with the intention of providing whole family case management.
The Care Coordinator Supervisor will be responsible for a small caseload of Pathway of Hope and/or Health Home Care Management cases and will support program growth in the assigned region. Position requires regular, weekly travel for case management and team supervision.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide and document weekly supervision to a maximum of 8 direct reports.
• Ensure subordinates complete required trainings in program models and database systems
• Provide and support ongoing skills training and supervision of staff team to deliver standards of care that meet the expectations of Pathway of Hope, Children’s, and Adult Health Homes .
• Become expert-level competent in models of care to provide quality supervision, guidance, and training to staff team on integrating the models and support the whole family.
• Track, monitor and comply with all billing and monthly reporting activities to ensure full billing by care managers and/or manage Pathway of Hope grant reports. Complete monthly audits of assessments, care planning and care management documentation. Complete monthly statistics reports.
• Actively initiate referrals for care coordination to ensure caseloads for team members are at capacity and support program growth
• With Corps Officer leadership, coordinate and strengthen the integration of both the Pathway of Hope and Care Management models into local unit activities. Example: Coordinate with Corps Officers around marketing of programs, networking opportunities and implementation at the local level.
• This position is a member of the Divisional Department of Social Services and is responsible for supporting the expansion and delivery of the Divisional Pathway of Hope and Health Home Care Management programs.
• Provide Health Home Care Management and/or Pathway of Hope services directly to a reduced caseload.
