FlexStaff is seeking a highly skilled and experienced Regional Home Care Field Supervisor for our client to oversee clinical services in our participants' homes. In this crucial role, you will monitor and support nursing standards of excellence, ensuring the highest quality of care is delivered. The ideal candidate will collaborate with various departments, guide nursing personnel, and participate actively in the operational management of the organization.
Responsibilities:
- Oversee and supervise clinical nursing services provided in home settings, ensuring adherence to care plans and quality standards.
- Assess and evaluate the health care needs of participants, considering various factors that affect their well-being.
- Conduct joint visits with nurses for supervision and competency assessment.
- Train and guide nursing staff and aides in best practices of home health care.
- Collaborate with the Care Planning Team to develop and maintain comprehensive care plans.
- Maintain timely documentation and compliance with all regulations.
- Conduct routine chart audits and field visits as necessary.
- Educate participants and aides on medications, safety, and diet as required.
- Develop and foster positive relationships with co-workers, participants, and community resources.
- Stay updated on current health-related issues and evidence-based practices.
Qualifications:
Education:
Degree from an NLN-accredited nursing school (RN, BSN required). A Baccalaureate degree is highly desired.
Experience:
- Minimum of one (1) year of experience in home care as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program, and/or Licensed Agency.
- Experience with frail or elderly populations is required.
- Experience in a PACE Program or Managed Long-Term Care (MLTC) is preferred.
- Excellent organizational, communication, and presentation skills.
- Proficient in computer use and technology.
- Valid New York State driver's license and reliable transportation are preferred.
- Knowledge of New York State Department of Health Regulations.
- Current active and unrestricted RN license in New York State is required.
