Major Responsibilities \n\nProvide ongoing education, mentorship, and leadership development to facility MDS Coordinators.\nTravel required to various facilities.\nOversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.\nMonitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members. \nProvide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results. \n\n

Minimum Qualifications\n\nMust possess a current, unencumbered, active state license to practice as an RN. \nExperience in Skilled Nursing\/Rehabilitation facilities. \nPreferred experience in a multi-site, regional capacity. \nTwo (2)-Three (3) years of experience as a MDS Coordinator.\nRAC-CT or RNAC preferred.\nLeadership and supervisory experience preferred. \nYou must be qualified, compassionate, and dedicated to a job well done.\n\n
