Summary:

Vighter LLC is seeking to hire multiple Regional Pediatric Advisors. The Regional Pediatric Advisor works to assure delivery of quality healthcare services in accordance with the SOW for the medical services contract within medical units.

Principle Accountabilities:

Serves as the on-site regional pediatric medical authority for the medical services contract and medical units at for the U.S. Customs and Border Protection sites including, Office of Field Operations (OFO) ports of entry, and/or U.S. Border Patrol Border (USBP) Stations providing supervision, oversight, consultation, coordination, and direction for pediatric medical services, to include emergency care if needed. Provides pediatric medical direction and necessary guidance/oversight for appropriate advanced practice pediatric provider supervision, oversight, consultation, coordination, and direction for all medical services, to include pediatric mental health in the absence of a behavioral health advisor. Clinical supervisory authority over pediatric mid-level providers, as assigned. Provides on-call pediatric coverage for assigned medical units/sector 24 hours a day/7 days a week, however, call coverage may rotate based upon staffing. Responsible for developing and coordinating all aspects of pediatric healthcare delivery at his/her assigned facilities. May provide direct care to patients in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges. Provides age-appropriate screening, treatment, and patient instruction pertinent to the discipline and facility setting for acute and chronic conditions for pediatric patients. Maintains non-involvement in the security aspect of patient care and custody. Acts as an initial reviewer for all outside pediatric referrals/consults to specialty services, to include hospital or emergency care that exceed services offered at the medical unit. Audits referrals and post-referral follow-up, as needed, as part of the medical quality management program. Provides oversight for all pediatric patients requiring enhanced medical monitoring, and completes rounds as needed. Seeks guidance from/collaboration with the Medical Director, Deputy Medical Director, Behavioral Health Advisor, Physician Supervisor, or other leadership as appropriate. Serves on the national and local medical quality management committees and oversees pediatric focused root cause analyses and quality improvement projects/initiatives in collaboration with the national and local medical quality management committee. Performs record keeping functions in accordance with program policies and position. Maintains functional proficiency and ease of use utilizing electronic health records as required by, and appropriate to, position. Completes all initial, annual, and ad hoc training or competencies as required/assigned. Maintains patient confidentiality, and confidentiality of medical records, in compliance with the Privacy Act and HIPAA regulations in all work activities. Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII). Adheres to, and maintains awareness of DHS and CBP Policies, Procedures, Directives, and Operational Memoranda. Leads, attends, and participates in general/medical staff meetings at the local and national level, providing pediatric focused guidance and oversight, as needed. Performs pediatric chart reviews as part of medical quality management, focused professional practice evaluation (FPPE) for APPs, on-going professional practice evaluation (OPPE) for APPs, collaborative annual performance evaluation for APPs, pediatric based medical sentinel event reviews, quality assurance and perform improvement initiatives with pediatric focus. Completes summaries of all completed pediatric chart reviews, identifying trends, performance concerns, best practices. Management and oversight of the Peer review program for APPs assigned as part of OPPE for assigned APPs. Performs a mid-term review of performance with assigned APPs as part of an annual performance evaluation process, addressing any clinical practice concerns if identified. Provides emergency care to employees or visitors for life, death, loss of sight/limb situations until Emergency Medical Services Arrives, as needed. Functional proficiency and ability to document patient-centered health care in the required electronic health record or electronic medical record, in the required format, in a timely manner, and in compliance with contract policy and guidance. Licensure/Certification:  

Candidates must have a current, valid and unrestricted license or registration as a Doctor of Medicine (M.D.) or as a Doctor of Osteopathy (D.O.). Licensure must be maintained current, and any required education or trainings to maintain licensure must be completed within the required timeline of the licensing authority. A temporary license will not be accepted. Candidate must not have had any professional licensure or certification withheld, suspended, revoked, or denied, and the candidate must not have surrendered or allowed a professional license or certification to have expired or lapsed because of an investigation or disciplinary action. Candidate must not have been convicted of, pled guilty to, pled nolo contendere to, or received probation before judgement for any crime other than a minor traffic violation, the record of which has not been expunged. Current unexpired Basic Life Support (BLS) Certification must be maintained from one of the following approved organizations: American Heart Association (AHA) BLS for health care providers American Heart Association Advanced Cardiac Life Support (ACLS), if applicable American Red Cross CPR/BLS for the professional rescuer American Safety and Health Institute (ASHI) CPR and AED for healthcare providers. Course components must include Adult, Pediatric, and Infant CPR, Foreign Body Airway Obstruction, and Automatic Defibrillator (AED) proficiency Applicant/ Employee must be certified upon hire and may also be a certified instructor BLS must be renewed prior to expiration date, and always kept in an active status Current board certification in Pediatrics by the American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA) approved board is required. Bilingual in English and Spanish encouraged, but not required. 

Education:  

Degree: Doctor of Medicine, Doctor of Osteopathic Medicine or equivalent from a school in the United States or Canada. The degree must have been accredited by the Council on Medical Education of the American Medical Association, Association of American Medical Colleges, Liaison Committee on Medical Education, Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Medical School: A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or completion of the U.S. Medical Licensing Examination.

Qualifications:

Must have a minimum of five years of clinical experience in direct patient care as a registered nurse and at least three years of experience in risk management, patient safety, and quality management.  Experience in detention/correctional healthcare, ambulatory care, or a pediatric care setting encouraged, but not required.  Supervisory experience preferred, but not required. 

Duty Hours / On-call Requirements:

Generally speaking, locations supporting the CBP-ICE mission set are open and active 24/7/365, to include days, nights, weekends and all holidays. We try our best to accommodate your schedule, so please inform Vighter of your desired schedule and/or geographic limitations in advance.

Travel:

As required to satisfy the contractual obligations. Generally, this is a regional assignment that requires working from a “home base” (not “work from home”) and traveling to numerous CBP locations within the assigned region. Commute is non-compensatory. All official travel is government-funded.

Physical Requirements:

Must be able to sit for more than 8 hours per day. Must be willing to work overtime if necessary. Must be able, with advance notice, to float to sites within 20 miles. Must be able to use an elevator or walk up and down stairs. Must be able to lift up to 40 pounds and carry up to 10 feet. Must be able to routinely grasp or handle objects, use finger dexterity, bend elbows/knees and reach above and below shoulders. Must be able to read and interpret handwritten and typewritten print. Must be able to communicate by voice and detect sound by ear. Must be able to perform duties in an indoor or outdoor environment. Must be able to pass a criminal background investigation. Must undergo Urine Drug Screening (UDS). Positions require applicants to be US Citizens, 21 years or older.  Must complete criminal background check and Child Abuse and Neglect (CAN) background check.
