**Experience & Requirements** 1. Bachelor’s and Master's Degree in Social Work, Case Management or closely related field. Master’s level and clinical license highly preferred. 2. Experience in case management, discharge planning and providing services to a physically and mentally ill population preferred. Prior corrections or re-entry experience is also desirable. 3. Experience using Excel, Word, and Outlook. 4. Effective time management skills 5. Other duties as assigned by management 3. Responsible for developing strategic planning activities to ensure YesCare Corporation is utilizing its re-entry resources in a successful manner. 4. Build collaborative relationships with organizations and providers in the community partnerships and MOUs. 5. Development of linkages and referral relationships between the Philadelphia Department of Prisons (PDP) regional facilities and local community-based organizations, primary care providers, community mental health/developmental disabilities centers, substance abuse programs, and crisis response teams to ensure continuity of care after detainee is released from (PDP) regional facilities. 6. Works collaboratively with the medical and mental health staff, (PDP) regional facilities Court Liaison, and the (PDP) regional facilities Inmate Programs to facilitate the discharge planning process and achieve the goal of a successful transition and re-entry to the community or/alternative sentencing. 7. Develops a multi-systemic needs analysis that documents external linkages and matches the needs with available community support. 8. Develops, maintains, and updates a community needs assessment for integrated care that involves discharge planning and successful reentry to the community. 9. Assists in maintaining Re-entry website/portal contents current. 10. Documentation of encounters is done in the healthcare record. 11. Monitors discharge-planning activities and results. Monitors and recommends strategies to reduce recidivism rates of discharged individuals. 12. Oversight and management of Norristown State Hospital process. 13. Maintain confidentiality and other standards as set forth by HIPAA, NCCHC, and other State and Federal guidelines. 14. Develops strategies to improve access to healthcare for discharged detainees. Develops CQI studies in collaboration with HSA and DON to determine and increase effectiveness of YesCare’s re- entry and recidivism reduction processes 15. Participates in any committees/meetings as deemed necessary by (PDP) regional facilities and/or YesCare. 16.
