**Position Highlights**
**SUMMARY:** The dietitian provides individual and group diabetes self-management education and serves as a resource to other professionals. Supervises menu planning and provides dietary staff education. Consults on dietary needs when needed on all patients and residents including patients receiving swing bed/skilled care services.
**EDUCATION and/or EXPERIENCE:**
+ Bachelor's degree required. Two years of experience in dietetics with experience in diabetes education preferred.
**CERTIFICATES, LICENSES, and/or REGISTRATIONS:**
+ Must be a Registered Dietitian with the Commission on Dietetic Registration (CDR) or obtain registration within three months of hire date is required. `
+ Licensed as a Dietitian in the State of Iowa.
+ Certified Diabetes Educator (CDE) preferred.
**ESSENTIAL DUTIES AND RESPONSIBILITIES**
Assesses patient learning needs and nutritional status and provides diabetes self-management education consistent with appropriate principles of teaching/learning and according to the American Diabetes Association Recognition and American Dietetic Association requirements. Plans the delivery of medical nutrition therapy and implement an individualized outcomes based nutritional care plan specific to the patient’s needs with clearly defined, measurable goals. Functions as an integral member of an interdisciplinary health care team (including physicians, diabetes team members, other hospital staff and other health care agency staff) regarding the care and education needs patients. Plans, conducts and evaluates patient, community and health professional education programs. Maintains ongoing communication with the Dietary Supervisor related to relevant Food and Nutrition Services policies and procedure, professional issues and any matters related to quality care for all patients. Demonstrates knowledge of age-specific factors and performs satisfactorily related to diabetes patient’s age infant through geriatric. Plans and initiates professional development responsibilities. Required to maintain continuing Education requirements set by state licensure regulations and at the Commission on Dietetic Registration, and develop professionally in the area of assignment.
