Job Description:

Under limited supervision, provides professional diabetes medical nutrition therapy to groups and individuals of all ages and abilities, in a variety of settings in the outpatient diabetes service area. Follows established standards of practice and guidelines defined by the American Diabetes Association (ADA) and Academy of Nutrition and Dietetics for the assessment of patients, and subsequent planning, implementation, evaluation and continuum of care for out-patients requiring diabetes medical nutrition therapy. Must have comprehensive knowledge of current ADA diabetes self-management education principles. Also provides support the outpatient nutrition education services as needed.

Bachelor's DegreeKentucky LicenseCertified Diabetes Care and Education Specialist exam NCDBE eligibility within 6 months
