Summary:

The Diabetes Center Registered Dietitian, CDE is responsible for assigned activities in the area of outpatient nutritional care and diabetes self management. Assures nutritional adequacy and appropriateness of individual diets specifically related to the diabetic patient. Interviews, evaluates, and counsels individuals on general and modified diets. Serves as a source of nutrition information for the facility and the community. Shares responsibility for planning, implementing and controlling work process. Works collaboratively with physicians on promoting diabetic education. Case manages select diabetic groups as directed. Performs organizational and community inservices. Manages a specific patient volume. Upholds education requirements needed for a quality educational program.

 

Experience/Education Requirements:

Four year baccalaureate degree from an accredited college/university with a major in dieteticsExperience in Diabetes Education preferred

 

License or Certification Requirements:

American Dietetic Assocation RegistrationIL state licenseCertification in Diabetes Education required through the National Certification Board for Diabetes Education obtained within 2 years of hire

 

 

Responsibilities:
Acts as a resource for diabetes and nutrition information for the facility and the community, maintaining knowledge base of new treatments in diabetes.Assesses inpatients for outpatient enhanced learning.Assesses patients including blood pressure, foot exam and waist circumference.Collaborates with physician regarding appropriate nutritional/medical management of the diabetic patient.Demonstrates leadership skills as a diabetes nutrition expert by collaborating with other health professionals to facilitate changes in patient care delivery.Effectively interviews, evaluates, and educates individuals with diabetes on appropriate modified diets and diabetes management.Performs organizational and community inservices.Provides accurate and timely documentation of services provided via medical record and prepares report of physician.Provides monthly reports to the Director as assigned.Reassesses patient learning needs via active case management.Shares responsibility for planning, implementing and controlling work process.Upholds education requirements needed for a quality education program.

TB or Airborne Pathogens: There is a potential for exposure from infected patients, (over 40% of population is geriatrics). This can be reduced by adherence to universal precautions.

Speech: Needed for presentations/training, telephone communication, facilitate meetings.
Vision: Needed to read memos and literature, doctor's orders, perform foot exams and teach self-blood glucose monitoring.
Smell: Needed to smell alcohol ingestion by patient, ketosis detection in sick patients with diabetes.
Hearing: Necessary to hear patient concerns; needed to hear alarms, and phones.
Touch: Need to write, do computer entry, filing, perform foot exams and teach self-blood glucose monitoring.
