Registered Nurse, Primary Care at Home, Wilmington, DE
Job Details

ChristianaCare is currently seeking a full-time Registered Nurse for our Primary Care at Home position to cover patients residing in the North Wilmington area.

In this role, you will be responsible and accountable for a group of patients and provide care to these patients in their homes as well as Assisted Living Facilities through the nursing process in collaboration with other healthcare team members as a professional care provider.

WORK SCHEDULE

Monday-Friday, 8:30AM - 5:00PM

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Travel to patient's homes and other facilities will be required.Implements nursing care in a knowledgeable, skillful, consistent, and continuous manner.Functions to establish priorities of patient care based on essential patient needs and available unit resources of time, personnel, equipment, and supplies.Assists providers with procedures.Performs various lab / point of care tests; reads and records results.Accurate and complete medication reconciliation and help patients manage their medication safely and effectively.Document in electronic medical record (EMR) system in a timely and accurate manner.Functions with an awareness and application of safety policies and in compliance with Infection Control policies.Performs effectively in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events.Demonstrates sound knowledge base and actions in the care and decision making for designated patient population.Demonstrates personal responsibility and accountability for own professional practice at all times.Demonstrates awareness of legal issues and patient rights in all aspects of patient care and strives to manage situations in a reduced risk manner.Actively participates in the development of department, and Nursing goals.  Maintains required credentials of CPR, mandatory inservice education.Demonstrates effective communication methods and skills, using lines of authority appropriately.Formulates and maintains effective working relationships with all health care team members.Maintains an attitude of helpfulness, courtesy, and cooperation towards patients, families, physicians, and other departmental/hospital personnel.Demonstrates skills and knowledge necessary to provide care to adult patients, including the ability to obtain and interpret information to identify patient needs, and to provide the care needed.                             Performs assigned work safely, adhering to established departmental safety rules and practices.  Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.Performs other related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:

Two years of experience as a RN

SPECIAL REQUIREMENTS:

Current RN licensure in the State of Delaware.CPR certification.Proof of a valid driver’s license and active auto insurance is required.
