We are so glad you are interested in joining Sutter Health! **Organization:** SCAH-Sutter Care at Home - Bay **Position Overview:** The Team CWOCN serves as a clinical resource for wound and ostomy related care issues for clinical staff, administrative staff, Quality department, and materials management department. The Team CWOCN provides direct patient care for complex wound and ostomy patients as directed. **Job Description** : **EDUCATION** + _Equivalent experience will be accepted in lieu of the required degree or diploma._ + Bachelor's: Nursing + Other: Completion of a accredited Wound, Ostomy, and Continence Program with certification in wound and ostomy. Continence highly preferred **CERTIFICATION & LICENSURE** + RN-Registered Nurse of California + CWOCN-Certified Wound Ostomy Continence Nurse + DL-Valid Drivers License Class C + AUTO-Automobile Insurance + BLS-Basic Life Support Healthcare Provider + Continence certification highly recommended + Current PPD **PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:** + 1 year of recent, supervised experience as a Wound/Ostomy nurse in acute or home care setting is strongly preferred. + 1 year experience in a home health position preferred. **SKILLS AND KNOWLEDGE** + Exhibits excellent oral and written communication skills; legible handwriting for documentation of patient records in a clear and concise manner. + Basic understanding of home care services, clinical standards of care and practice; must have clinical knowledge and critical thinking ability to effectively plan and provide patient care consistent with the standards and regulations of Title XXII, Medicare conditions of Participation and home health accreditation requirements, QI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payers. + Must be able to follow oral and written instructions. + Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, and to effectively solve unique problems as they arise or identify when to consult supervisor. + Must be able to deal with a challenging work environment from time demands and conflicting priorities. + Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements. + At orientation and annually thereafter, must be able to meet core competencies for position and area of specialty, as appropriate. + Ability to be aware of surroundings and respond to emergency and/or urgent events. + Must be detail-oriented. + Self-starter with a high degree of initiative, motivation, flexibility, energy, and creativity. + Ability to form harmonious working relationships with internal and external customers. + Demonstrated leadership and negotiation skills and the ability to manager resources for patient care in a cost-effective manner. + Excellent collaborative and problem solving skills with customers and management. + Ability to exercise independent sound judgment in planning and providing patient care. + Demonstrated ability in physical, psychosocial, and environmental assessment skills and in implementing plan of treatment + Ability to communicate with possibly disoriented, combative patients. + Ability to work in mentally and physically demanding environment. + Excellent organizational skills. + Access to a reliable motor vehicle to travel to patient sites. **Job Shift:** Days **Schedule:** Full Time **Shift Hours:** 8 **Days of the Week:** Monday - Friday, Variable **Weekend Requirements:** As Needed **Benefits:** Yes **Unions:** Yes **Position Status:** Non-Exempt **Weekly Hours:** 40 **Employee Status:** Regular Employees of Sutter Health and its entities may handle hazardous drugs in the course of their work, including patient care, which requires them to manage, store, prepare, receive, unpack, transport, dispose of, or administer drugs identified as hazardous or potentially hazardous by the National Institute for Occupational Safety and Health (NIOSH) and in accordance with the USP 800 guidelines. Pay Range is $68.91 to $92.60 / hour _The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
