**Part Time,Day Shift** Mon – Fri
Responsible for organizing and coordinating the stroke program and acts as subject matter expert in the Quality/Patient Safety Field; to support sections of the Quality and Patient Safety Program at Exeter Hospital. This position reports to the Director of Quality.
**Requirements:**
+ 3 to 5 years of Directly Related experience
+ Accredited RN Program
+ RN license ‑ NH or other Compact State
+ ACLS, PALS
+ Strong communication and analytical skills
+ Specialty certification (CEN) preferred
**Responsibilities:**
1. Provides support, coordination, and oversight for the Stroke Program. Coordinates and facilitates data collection for purposes of performance improvement and standard of patient care, implements QI projects. Extract, populate, and organize data collection for the Multidisciplinary Stroke Task Force database. Acts as subject matter expert and resource for Stroke.
2. Performs real time chart audits, provides support and is a resource for stroke care. May assist with facilitation and coordination of care for stroke patients throughout the hospital; participates in rounds on inpatient strokes. Proposes developmental plans for areas of improvement.
3. Co‑leads the Multidisciplinary Stroke Task Force meeting and case reviews on a monthly basis. Meets monthly with the NH Stroke Collaborative group and quarterly with the NE Stroke Coordinator Alliance. Provides consultation and education to staff and provides Community Stroke Education. Prepares educational opportunities, participates in presenting and educating staff at minimum of four hours annually.
4. Liaison for communication, mediation and case review with Massachusetts General Hospital Tele‑Neurology Services.
5. Provides timely feedback to members of the Stroke Committee with regard to stroke care protocol performance and other performance indicators and develop presentations for reporting quality data.
6. Develops and disseminates best practices, information related to protocols, tools and templates, determined and approved by the Multidisciplinary Stroke Task Force and Stroke Champions.
7. Maintains Policy and Procedures, coordinates certification/recertification and other accreditation activities.
8. Performs similar or related duties and staff nursing functions as requested or directed for stroke and quality programs.
9. Quality Reporting – Oversees collection, analysis and reporting of the key measures required for incentive payment, regulatory and accreditation program compliance; educates team members re: the technical specifications of measurement and quality reporting to ensure organization-wide performance is at the highest level possible for optimal reimbursement and bonus payments.
10. Data Management- Selects, defines and obtains process and outcome data for departments to monitor and understand quality performance, set improvement goals and evaluate the results of improvement efforts. Builds scorecards and reports, and provides information related to individual staff and provider activities to support peer review, Quality Improvement Priorities and accreditation requirements. Creates and uses statistical charts and other industry tools within the data analysis process.
