Description:Job Summary

Trinity Home Health is seeking a compassionate and dedicated Registered Nurse (RN) to provide high-quality patient care in the home setting. The RN will be responsible for assessing, planning, implementing, and evaluating nursing care for patients in their homes, ensuring they receive the best possible care while promoting their independence and well-being.

Key ResponsibilitiesPatient Assessment: Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social needs.Care Planning: Develop individualized care plans in collaboration with patients, families, and the healthcare team to address specific health needs and goals.Clinical Care: Provide direct nursing care, including administering medications, managing wound care, and monitoring vital signs and symptoms.Education: Educate patients and families on disease management, medication administration, and health maintenance to empower them in their care.Collaboration: Work closely with physicians, therapists, and other healthcare professionals to coordinate care and ensure optimal patient outcomes.Documentation: Maintain accurate and timely documentation of patient assessments, care plans, and progress notes in compliance with regulatory requirements.Advocacy: Advocate for patients’ rights and preferences, ensuring they receive the highest standard of care in their home environment.QualificationsEducation: Bachelor’s degree in Nursing (BSN) preferred; Associate’s degree in Nursing (ADN) accepted.Licensure: Current, unrestricted Registered Nurse (RN) license in the state of [Insert State].Experience: Previous home health or community health experience preferred but not required.Skills: Strong clinical assessment and critical thinking skills, excellent communication and interpersonal abilities, and the ability to work independently.Certifications: Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) certification preferred.What We OfferCompetitive salary and benefits package, including health, dental, and retirement plans.Opportunities for professional development and continuing education.A supportive and collaborative team environment focused on patient-centered care.
