SUMMARY
The Registered Nurse (RN) (a) assists each resident in achieving and maintaining their highest practical level of physical, mental, and psychosocial well-being through providing medication, treatment administration and quality, personal care. The RN utilizes the nursing process in planning, directing and providing direct care to residents in accordance with professional nursing practice standards, established policy and procedures, individual plan of resident care and regulatory requirements.
PRIMARY RESPONSIBILITIES
1. Provide direct care to residents under individual plans of care, administering care and medications per doctor’s orders.*
a. Act as an advocate to maintain client privacy and dignity during cares. Administer prescribed medications and treatments per doctor’s orders. *
b. Monitor resident vitals, completing resident assessments as assigned. Communicate any identified changes to physician and family members. *
2. Collaborate with the IDT (interdisciplinary team) to ensure adequate and appropriate flow of services. *
3. Supervise dining room activities, assisting residents with eating as needed.
4. Assist throughout the admission process, to ensure smooth transition of new residents. *
5. Periodically perform the comprehensive assessment of residents with acute changes of condition, and attend resident Care Conferences as needed.
6. Oversee and lead nursing staff, including LPNs, NARs, and TMAs. *
a. Communicate expectations and ensure that unit personnel are carrying out their assignment. *
b. Organize and provide work direction to unit staff. *
c. Facilitate conflict resolution between residents, staff, families, community and other disciplines (Hospice, Therapy, M.D.s, Nurse Practitioners, etc.) *
7. Serve as a clinical resource to staff regarding nursing care of residents. Provide in-service education as requested.
8. Maintain and complete all required documentation and charting on the computer or on forms in an accurate and timely manner according to policies and procedures. *
a. Accurately transcribe Physician’s Orders as per facility protocol and forward necessary information to other departments as needed.
b. Initial Medication Administration Record (MAR) after medication is administered per facility policy and procedure, accurately documenting refusals for medication on back of MAR. *
9. Support various other Nursing responsibilities as needed.

JOB SPECIFICATIONS
Education, Experience and Credentials
· Current Registered Nurse licensure in the State of Minnesota required.
· Current CPR certification required.
· One or more years’ experience in a long-term care setting preferred.
· Strong clinical assessment experience preferred.
Knowledge, Skills and Abilities

· Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.
· Possess effective problem solving skills.
· Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping.

· Able to understand and follow facility and department safety and emergency procedures.
· Understand and maintain confidential nature of organization and client information in accordance to HIPAA regulations and corporate compliance standards, including appropriate access of data housed within the Information Technology infrastructure.
PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB
Incumbent must be able to:
· Perform medium work, lifting up to 50 pounds, and occasionally lifting/carrying of objects weighing up to 25 pounds.
· Push/pull people and equipment up to 50 pounds a distance of several feet.
· Twist, turn, bend, stoop and reach frequently in the performance of client assistance responsibilities.
· Effectively communicate in the English language, including following oral and written instructions, in order to communicate with people inside and outside the organization.
· Able to effectively use office equipment to review and create information.
· Work at a desk for extended periods and perform work at a computer.
· Frequently stand and walk to move around office/building repeatedly throughout the day.
· Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.
· Must have reliable transportation.
TOOLS AND EQUIPMENT USED
· Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.
· Use telephone, computer and standard office equipment.
WORK ENVIRONMENT
The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of residents, in responding to emergencies. May be at risk of exposure to blood, bodily fluids, and other potentially infectious material.
