Job Description

Under the direction of a Nurse Manager, the PACE Community Health Nurse (PCHN) provides skilled care and routine clinical nursing assessment visits to PACE members and families, utilizing all elements of the nursing process and in compliance with applicable laws, regulations, and agency policies. The PACE CHN is a core interdisciplinary team (IDT) member assigned to a PACE member in compliance with CMS PACE regulatory requirements.

ResponsibilitiesCarry out the agency’s mission, philosophy, goals, and objectives within guidelines of Agency policy and position function.Interpret and implement the Agency’s philosophy to staff and members of the community.Assume responsibility for assignments given, seeking supervision appropriately and performing within the limits of their education and experience.Conform to the law governing the practice of professional nursing and provide professional nursing care using all elements of the nursing process.Participate in mandatory PACE comprehensive care planning meetings routinely as required by PACE regulations.Collaborate with all members of the IDT in developing a comprehensive care plan based on member-specific needs, physician orders, UAS-NY CAPS, other IDT assessments, members’ specific parameters, and identified goals that are respectful of member, family, community, and agency resources.Delegate responsibility appropriately and supervise ancillary personnel in a manner that will assure quality care and compliance with the care plan.Complete, maintain, and submit all required documentation that is timely, accurate, and relevant.Only act within the scope of the individual’s authority to practice.Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.Essential SkillsCurrent New York State license to practice as a Registered Nurse.Minimum of one (1) year general clinical nursing experience in a certified home health agency, acute care, medical surgical, and/or critical care experience.Clinical nursing experience must be within one (1) year from the date of hire or transfer.Minimum of one (1) year of experience working with a frail or elderly population, or appropriate training from the PACE organization.Possess and maintain good physical stamina and mental health, including satisfactory health clearance as required by NYS regulations and Agency policy.Current Driver’s License, required insurance, and car available for work as required.Additional Skills & QualificationsBe legally authorized (licensed, registered, or certified) to practice in the State in which the healthcare professional will perform the function.Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.Computer/laptop proficiency required.Good organizational, observation, communication, and judgment skills.Self-directed with the ability to work with supervision.Flexible and cooperative in fulfilling role obligations.Demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.Must be able to safely maneuver stairs, climb up/down, and walk to access work areas. Position requires the individual to be able to travel and walk between sites/locations and work areas throughout the day.Work Environment

The position entails a schedule of Monday – Friday 8:30 am-5:00 pm (37.5 hours), with occasional weekend work as needed, frequency varies per location. The work environment involves community health nursing, requiring travel and walking between sites and work areas throughout the day. The role includes working within a team setting, using electronic health records, and proficiency with Microsoft Word is required. The position is unionized under the 1199 Union.

Pay and Benefits

The pay range for this position is $105861.00 - $118061.00/yr.
