Dive into Patient Care: Assess patients’ needs from admission to discharge, creating and coordinating personalized care plans. Empower Through Education: Provide vital information and resources to patients and their families. Optimize Care: Monitor healthcare resources to enhance outcomes and reduce costs. Facilitate Smooth Transitions: Ensure timely and safe discharge plans, helping patients transition to the next level of care seamlessly.

We encourage you to apply if you:

EDUCATION/EXPERIENCE REQUIRED:

·       Have graduated from an accredited BSN program.

·       Possess RN licensure in the state of Michigan.

·       Possess a minimum of three (3) years of acute care/inpatient nursing experience.

·       Inpatient Case Management and/or discharge planning and care coordination experience strongly preferred.

·       Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem solving skills and excel at planning.

·       Are self-motivated and enjoy working in a fast-paced environment.
