Join Our Team as a Registered Nurse (Case Manager)!

Position Details:

Labor Category: Registered Nurse (Case Manager)Department: Naval Health Clinic Lemoore, CAHours: 8hrs a week, 40hrs weekly 

Qualifications:

Education: Baccalaureate Degree in Nursing, Graduate Diploma in Nursing, or Associate Degree in Nursing from an ACEN or CCNE accredited program.Certifications: Basic Life Support (BLS) from American Heart Association or American Red Cross (No cost to the government).Experience: Minimum of 1 year as a RN Case Manager within the past 3 years.Board Certification: Must possess one of the following:Certified Case Manager (CCM)Certified Disability Management Specialist (CDMS)Certified Rehabilitation Registered Nurse (CRRN)Certified Occupational Health Nurse (COHN) or COHN-SAdvanced Certification in Continuity of Care (ACCC)Certified Rehabilitation Counselor (CRC)Nurse Case Manager (RN-NCM)Care Manager Certified (CMC)Licensure: Current, full, active, and unrestricted RN license.

Key Duties:

Adhere to Joint Commission, URAC, and CMSA requirements.Utilize medical care criteria such as InterQual.Identify and manage high-risk patient populations.Provide case management consultation.Collaborate to establish and update case management care plans.Educate patients and families on care plans and decision-making.Measure and document patient responses to care plans.Coordinate multidisciplinary discharge planning.Evaluate patient, family, and provider satisfaction.Facilitate continuity of care during patient transitions.Support and educate other case managers.Plan for professional growth and participate in relevant professional organizations and conferences.Provide safe, quality clinical case management services.Stay informed on new methods and practices in case management.Assist in the ongoing improvement of the case management program.Perform clinical assessments and follow-up evaluations.Integrate case management with utilization and disease management as needed.
