Registered Nurse Clinical Care Manager Emergency Department

Description:

At St. Joseph’s Health the Emergency Department records more than 70,000 patient visits a year.  As A community-centered emergency department, it offers a wide range of medical services for people of all ages.  The Registered Nurse Utilization Review (UR), Clinical Care Manger (CCM) actively manages the utilization review process and patient progression from Admission to Discharge. Through partnership and collaboration with all members of the healthcare team, the UR/CCM is the principal liaison between emergency services, surgical services, nursing, finance, physicians, and medical records. In addition, they are responsible to coordinate safe and appropriate discharge plan for the patients within their service area. 

What you will do:  

Patient Care Coordination: Completes initial and follow up case reviews on all patients as dictated by payer and status. 

Leads daily SNAP huddles and works collaboratively with unit staff and support team (RN, Physician/Clinical Affiliate, DPA, MSW, CDI, Rehab Services, etc.) to progress each patient's care towards discharge. 

Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice. Recognizes situations that require immediate intervention. Continues to develop professionally and apply knowledge and skills necessary to provide care appropriate to each patient.  

Minimum Qualifications:  

Education: Graduate of an accredited college or university affiliated nursing program.  BSN graduate preferred.

Licensure: Current Licensure as a Registered Nurse RN in New York.

Experience: Knowledge of InterQual/Indicia/MCG software, Minimum of 3 years of clinical experience in an Emergency Department or acute care setting. Previous leadership experience is highly desirable.

Work Schedule:  Full time – Day Shift 40 Hours, 4-10’s or 36 Hours 3-12’s, Every 3rd Weekend, No Holidays

Pay Range:  $35.80 - $53.70

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
