At St. Joseph’s Health the Emergency Department records more than 70,000 patient visits a year. As A community-centered emergency department, it offers a wide range of medical services for people of all ages. The Registered Nurse Clinical Care Manager Emergency Department Manager is responsible for assessing, planning, coordinating, and implementing case management activities in the emergency setting. This role works collaboratively with physicians, nurses, social workers, and other healthcare professionals to enhance patient outcomes.
What you will do:
Attend SNAP huddles if needed and works collaboratively with unit staff and support team (RN, Physician/Clinical Affiliate, DPA, MSW, CDI, Rehab Services, etc.) to progress each patient's care towards discharge.
Provides patient and family education regarding care plans, discharge instructions, and available resources.
Documents case management activities accurately and thoroughly in the electronic medical record (EMR).
Participates in quality improvement initiatives aimed at reducing length of stay, preventing avoidable readmissions, and improving patient outcomes.
Minimum Qualifications:
Education: Graduate of an accredited college or university affiliated nursing program. BSN graduate preferred.
Licensure: Current Licensure as a Registered Nurse RN in New York.
Certification: BLS/CPR
Experience: Minimum of 2 years clinical experience in a hospital setting, and previous discharge planning, home care, and/or managed care experience
Work Schedule: Full time – Day Shift 40 Hours, Monday thru Friday 8 Hour Shifts, Every 12th Weekend, No Holidays

Pay Range: $35.80 - $45.35
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
