Registered Nurse Clinical Coordinator PT Days

Description:

Provides coordination and lateral integration for the Joint replacement and orthopedic patients between clinicians, management and patient care activities consistent with our MAGIS patient/family Centered Model of Care to achieve high standards of patient care and quality outcomes. Works with the multidisciplinary team including   physicians, staff, internal and external partners to insure patient's progress along the  continuum of care      in an efficient and cost effective manner that ensures quality outcomes. Utilizes  established evidence based guidelines to support appropriate level of care throughout the  hospitalization and beyond. Acts as  a patient advocate and communication link with other departments, and community resources.

Facilitates multidisciplinary rounds and supports nursing staff in development of individualized, evidence-based outcome oriented, safe and timely practices.

Works with the multidisciplinary team including physicians, staff, internal and external partners to insure patient's progress along the continuum of care in an efficient and cost effective manner that ensures quality  outcomes. Utilizes established evidence based guidelines to support appropriate level of care throughout the hospitalization and post-hospital care. Participate in facility clinical research trials as designated by the program medical director.

BSN required, AHA BLS required

BSN Pay Range: $34.76 - $57.06 per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
