Rehabilitation Nursing: Making A Difference in Every Life We Touch....

By the time a patient arrives in the care of a rehabilitation nurse, their life has already been saved. It's now up to the rehab nurse to restore that life to freedom and independence once again. Rehab nurses are poised to provide this restorative care through our expert knowledge and understanding of the holistic, individualized, and complex needs of each patient.

Rehabilitation nursing offers a gratifying career for nurses who want to use all their clinical skills, make a visible difference in their patient's and caregiver's lives, and work as part of a collaborative healthcare team. As a collaborator, educator, care coordinator, advocate, and change agent, rehabilitation nurses work as an integral part of the Interdisciplinary Team that includes physiatrists, physical therapists, occupational therapists, speech therapists, and psychologists to develop comprehensive care plans based on patient diagnosis, goals, and maximum potential.

ALS Amputees Brain Injury, Traumatic & Non-Traumatic Complex Wound Care Needs Cardiovascular Cerebral Palsy Guillain-Barre Syndrome Parkinson's Disease Major Joint Replacements Multiple Sclerosis Organ Transplant Pulmonary Disease Renal Disease, including Hemodialysis and Peritoneal Dialysis patients Spinal Cord Injury Stroke

Rehab nurses are poised to provide restorative care through expert knowledge and understanding of the holistic, individualized, and complex needs of each patient.

General Summary

Provides professional nursing care at the intermediate practitioner level in accordance with the principles of patient and family-centered care based on the nursing process and according to the clinical standards of the hospital. Coordinates care planning with other disciplines. Coaches and mentors the Clinical Nurse I. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. May require specialized skills depending on unit assignment.

Duties and Responsibilities

Essential Functions:

Meets all expectations of CNI. Coaches and mentors the CN I in developing and coordinating the primary nurse case load and their plans of care. Initiates and manages plans of care for more complex groups of assigned patients. Independently recognizes subtle changes in condition and modifies plan of care accordingly. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. Initiates emergency action in response to acute risk situations. Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload. Evaluates patient care and promotes goal-directed change to achieve patient outcomes. Able to discuss, document and apply Performance Initiatives and outcomes at the unit level as they relate to the WellSpan Wildly Important Goals (WIGs) and Nursing Sensitive Indicators (NSIs). Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients. Initiates interdisciplinary collaboration in evaluation of patient progress. Identifies and facilitates individualized patient needs for transfer/ discharge and includes patient/family in decisions. Begins reading current literature. Express interest in evidence based research and its application to best practice. Understands clinical relevance and its impact on patient outcomes. Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Required education and competencies completed, including 30 CEU's above system required education. Encouraged to share pertinent information and job knowledge, related to readings, educational programs and participation in professional organization activities. Meets mandatory criteria for CNAP II level. Actively promotes an environment conducive to professional practice, interdisciplinary collaboration, and patient and staff safety. Supports Shared Decision Making by unit and/or hospital-wide committee membership or by supporting and helping fellow co-workers attend committee meetings and/or work on SDM projects. Demonstrates leadership at point of care. Provides leadership for problem solving and goal formation. Applies current WIGs to care delivery. Begins to take on charge duties/responsibilities. Anticipates needs for alterations in staffing for upcoming 24 hours and assists with actions to adjust. Accountable for completion of required charge nurse responsibilities. Demonstrates ownership of patients beyond individual assignments. Applies comprehensive thought process. Promotes cooperation and communication among all team members. Seeks opportunities to assist co-workers without compromising individual responsibilities. Gives feedback to team members about unique strengths and contributions. Consistently interacts with others in a manner that is perceived to be positive, supportive, and respectfully delegates appropriately Flexible with schedule and assignments; adapts to changing unit conditions (admissions, discharges, emergencies, etc.). Displays conflict resolution and negotiation skills to maximize team performance. Provides feedback in a positive and respectful way. Aware of and investigates events that led to conflict; recommends preventative measures or solutions. Acts as coach to CNI staff for clinical issues and potential advancement opportunities. Variation of duties may result based on the assigned customer care unit(s). This may include differences in the specific clinical skills required of the RN. Unit based skill lists outline expectations beyond the house wide RN requirements. SYS-NURS-L Nursing Education Requirements Policy outlines current certification(s) required for specific areas of nursing practice and is considered to be part of the job description. In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required.Current certification may be required for the following areas of nursing practice:Emergency Care Center - ACLS, PALSIntensive Care Center & MSCC Lead - ACLS, PALSL&D - ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU - NRPPEDS - PALSPCC/PACU/ENDO - ACLS, PALSProcedural - ACLS

Common Expectations:

Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s). Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required. Enhances professional growth and development through advanced/continuing education to maintain up-to-date knowledge, skills, and competencies. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. Attends meetings as required and participates on committees as requested. Provides outstanding service to all customers using caring behaviors; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

Diploma Program Required or Associates Degree Required Bachelors Degree Nursing (BSN) Preferred

Work Experience:

2 years Relevant experience. Required

Licenses:

Licensed Registered Nurse Upon Hire Required or Registered Nurse Multi State License Upon Hire Required and Basic Life Support Upon Hire Required
