Klarus Home Health is a hiring dedicated and compassionate Registered Nurse Case Manager to provide nursing services and case management to our patients who receive home health services in the San Antonio, TX and surrounding areas.

The registered nurse plans, organizes and directs home health care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Requirements

Registered Nurse (RN) Responsibilities:

Follow a designated care plan in accordance with patient's needsMake home visits to clients in designated geographic territoriesPerform assigned duties, including administration of medication, wound care, treatments, and proceduresMonitor clients' conditions; reporting changes to Clinical or Client Services ManagerFollow up with, execute, and properly document doctors' ordersPerform client assessmentsCase management and coordinationAccurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications:

Current State or Compact Registered Nurse (RN) License in handA minimum of one year clinical experience is preferredDemonstrate knowledge and skillset in current nursing practicePossess a valid state driver's license and automobile liability insuranceHCHB EMR knowledge is desired
