JOB SUMMARY
The Registered Nurse – Home Health/Hospice is responsible for the provision of nursing care, and coordination of care provided by the agency team. The RN needs to have solid clinical expertise (assessment, critical thinking and clinical skills) to function independently in the home setting. Home Care and Hospice uses a Case Manager and visit nurse model. The RN utilizes the direct and extended health care team (Emergency Services, MD clinic, hospital, etc.) for safe care, and also collaborates with physician(s), patient/family, other health care providers or disciplines and applicable community resources to develop and carry out the plan of care. The RN actively participates in quality improvement activities and projects, which promote the services, and care of the organization. This position performs duties in the uncontrolled environment and dynamics of private homes and travel hazards; requiring strong critical thinking to assure safety.
Schedule: 8am-4:30pm Mon-Friday. Weekend and Weekday call rotation.
ESSENTIAL JOB FUNCTIONS
1. Maintains accurate day-to-day schedule, communicating changes promptly.
2. Participates in technology trouble shooting when needed.
3. Monitors for billing and insurance concerns, collaborating appropriately with office staff for resolution.
4. Completes assessments consistent with Home Care (OASIS) and Hospice (Comprehensive) requirements.
5. Facilitates, develops, and conducts ongoing assessments, goals, and interventions to address medical necessity for service.
6. Develops, implements, and facilitates patient-specific plan of care (POC) in collaboration with physician, patient/ family and other disciplines.
7. Triages patient/ family calls effectively, determining necessary course of action, and providing appropriate follow through and documentation.
8. Conducts drug regimen review in accordance with Conditions of participation.
9. Manages the patient’s plan of care from admission to discharge, including assigning coverage/care when unavailable.
10. Facilitate completion of Hospice Prognostication to assist Hospice Medical Director in determining recertification.
11. Coordinates care with internal and external disciplines and parties.
12. Educates patient and caregivers using patient specific approaches (verbal / written instructions, video, return demonstration, etc.), documents action and patient/ caregiver response.
13. Completes clinical record reviews as assigned.
14. Delegates appropriately, providing required supervision of CNA activities.
15. Regular attendance is required in order to carry out the essential functions of the position.
16. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.
ADDITIONAL DUTIES
1. Screens and accepts referrals in accordance with agency policy.
2. Orients and or mentors new staff and students.
3. Markets the agency, the organization, and/or health topics related to home care.
4. Facilitates disaster plan when indicated.
5. Participates in department, organizational projects, and policy development when needed.
6. Other duties as assigned.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Graduate of an accredited school of nursing.
Preferred/Optional: Bachelors of Science in Nursing.
EXPERIENCE
Minimum Required: None
Preferred/Optional: One year of nursing experience in a clinical setting.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required:
Current State of Wisconsin Registered Nurse License or Nurse Compact License.Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.Current state of Wisconsin driver’s license with acceptable record.Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
