**Job Summary and Qualifications**
**Role Summary:**
Responsible for providing direct patient care in accordance with physician’s orders, recognizing standards of practice and establishing company policies and procedures. Responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
**What you will do in this role:**
**Essential Accountabilities:**
Receives referrals for Hospice GIP referrals from hospital case management conducts intake process.
**Performance Criteria:**
+ Discuss GIP level of care with patient/family and completes consents.
+ Educate patient/family of need of discharge plan should patient stabilize. Offer resources and coordinate with MSW as needed.
+ Obtain necessary clinical document/information for GIP admission.
+ Assist hospital staff with transfer of chart and ensures all orders are in place.
+ Provides direct patient care to GIP level of care patients in hospital setting, evaluates outcomes and adjusts nursing care processes indicated to adhere to legal, ethical, and clinical standards.
**Performance Criteria:**
+ Formulates and updates plan of care based on patient assessment and identified needs.
+ Completes initial comprehensive and ongoing daily assessments for hospice patients.
+ Rounds on patients throughout the day to ensure symptoms are managed and offer support to patients and/or family members.
+ Implements current nursing practices following a comprehensive assessment plan of care.
+ Updates or modifies plan of care/goals as necessary to reflect the changing needs of the hospice patient in coordination with the hospice IDG team.
+ Follows the plan of care, provides treatments, and works collaboratively with the members of the team to help meet positive patient care outcomes.
+ Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
+ Povides follow-up on patient discharges, collects necessary clinical and demographical information to allow the clinical teams to determine appropriate level of care.
+ Documents in an accurate and timely manner, according to policy, procedure, and practice guidelines.
+ Collaborates and consults with patients, colleagues, and interdisciplinary team members to achieve desired outcomes.
**Performance Criteria:**
+ Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group, including Hospice Aide, Director of Clinical Services-Hospice, and Chaplain.
+ Provides input at interdisciplinary team meetings.
+ Documents and reports data, patient response and progress.
+ Individualizes patient teaching and care plan based on patient’s needs.
+ Serves as an advocate for patients and families.
+ Identifies problems and implements measures to meet needs of patients and families throughout the continuum of care.
+ Provides education for self, staff, colleagues, patients, and/or families.
**Performance Criteria:**
+ Serves as an education resource to the Hospital and community at large, regarding healthcare services. Coordinates available resources for staff and families. Educates on CarePartners Hospice to any appropriate audience.
+ Identifies hospital and hospice staff leaning needs related to the GIP level of care and process and implements plans to meet those needs, or communicates to appropriate person or department.
+ Provides teaching for bedside nurse on medications to be given and symptom management.
+ Precepts new staff as assigned.
+ Provides and documents individualized patient and family education.
+ Maintains unit/department specific credentials.
+ Acts as liaison between CarePartners Hospice and acute care hospital case management team, palliative care, and physician services.
**Skills, Knowledge, Abilities:**
Required Education: Diploma in Nursing, Associate’s Degree in Nursing, or Bachelor’s Degree in Nursing
Required License: Must have and maintain current licensure as registered nurse with the North Carolina Board of nursing. Compact license may apply, licensee should confirm with NCBON; BCLS. Valid Driver’s License
Preferred License: National certification in area
Required Experience: Minimum of one (1) year experience as a Registered Nurse
Preferred Experience: Minimum of one (1) year experience as Registered Nurse in hospice or palliative care setting.
