**Job Summary:**
JOB SUMMARY
Applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning. Effectively communicates pertinent patient/family information to the health care team. Collaborates in establishing patient goals and evaluates progress to ensure effective outcomes. Furthers professional growth by engaging in learning activities according to the Board of Nursing standards.

The Inpatient Virtual Registered Nurse works at an offsite location (Crystal Terrace) to provide care virtually to inpatient units across VUH. The inpatient virtual nurse is an integral member of the clinical bedside team and is responsible for coordinating the care of patients specifically with discharges and admissions. Other commonly performed tasks of the inpatient virtual registered nurse include safety rounding, dual medication verification/sign off, and mentoring/coaching of clinical bedside staff.
Key Responsibilities:
+ Collaborates and provides quality documentation with the multidisciplinary team, including Physicians, Registered Nurses, technicians, and other healthcare providers, for care coordination, admissions, education, high risk high alert medication verification, any pertinent virtual assessments, and discharges.
+ Utilizes clinical knowledge, critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient's needs and ensures a seamless, effective, and efficient transition of care across the continuum.
+ Exhibits a high degree of professional competence and autonomy in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, and 4) navigator.
+ Mobilizes resources to effect rapid and timely movement of the patient through the system to achieve targeted admission and discharge times established by organizational policy and procedure.
+ Actively participates in department meetings and operations, including process development or improvement (e.g., department orientation, internal mentor/training programs and initiates, disease and population management strategies, appropriate measures for evaluation of outcomes) and establishment of department goals, objectives.
+ Ensures evidence-based care and continuously monitors patient outcomes.
+ Recognizes complex learning needs and prepares individual teaching plans for patients/families and consistently demonstrates effective patient education to peers.
