RN Long Term Care Job Description
JOB SUMMARY
The RN Long Term Care is accountable for the provision and promotion of quality patient services and practices within the scope of the Wisconsin Nurse Practice Act, using technical and critical thinking skills and established standards to coordinate care of the residents and manage the facilities nursing staff.
Hours: 2:30pm-11:00-pm, every other weekend and holiday rotation
ESSENTIAL JOB FUNCTIONS
Assessment:
1. Assesses geriatric syndromes common to older adults, including: cardiovascular, respiratory, gastrointestinal, urinary, hematological, musculoskeletal, endocrine, neurological, psychiatric/psychosocial, integumentary, and sensory and pain problems. Resident Care:
2. Supervises the day-to-day nursing activities performed by the nursing assistants. Such supervision must be in accordance with current federal and state standards, guidelines and regulations that govern the facility to ensure that the highest degree of quality care is maintained at all times.
3. Ensuring that personnel providing direct care to residents are providing such care in accordance with the resident’s care plan and wishes.
4. Provides leadership to staff by monitoring and participating in hourly rounding.
5. Consults and coordinates with health care team members to assess, plan, implement, and evaluate resident’s care plans.
6. Prepares and administers medications as ordered by the physician in accordance with the facility’s policies and procedures.
7. Processes physician’s orders in accordance with facility procedures. Communication:
8. Clearly and concisely reports and communicates with other team members.
9. Reports changes in resident status to RN Supervisor and other team members.
10. Participates with keeping family members/POA members updated on resident’s current condition.
11. Participates in care plan/admission/discharge/behavioral meeting for the residents.
12. Participates with making rounds with physicians, physician’s assistants, and nurse practitioners.
13. Notifies the resident’s physician when there is an incident involving the resident or change in the resident’s condition.
14. Communicates issues of concern to the Director of Nursing, RN Lead or RN Manager. Documentation:
15. Documents completely, accurately and timely according to department guidelines using Electronic Charting System (ECS).
16. Safeguards patient confidentiality in verbal, written and computer documentation.

17. Utilizes sound principles of body mechanics.
18. Uses standard precautions: uses Personal Protective Equipment (PPE), appropriate linen and waste disposal per policies and procedures.
19. Assures that staff operates in accordance with Federal and State Regulations. Leadership:

21. Coaches and develops licensed and non-licensed nursing staff.
22. Leads and/or participates in implementation of change to provide high quality of care while maintaining cost effectiveness (Quality Assurance and Process Improvement (QAPI) Programs).
23. Mentors staff in new roles.
24. Assists resident, families and staff in problem solving and conflict resolution.
25. Regular attendance is required in order to carry out the essential functions of the position.
26. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.
ADDITIONAL DUTIES
1. Assists the scheduler in coordinating staffing needs and updating the schedule as necessary.
2. Serves on appropriate organizational wide committees as appropriate.

Nurse Tech Job Description
JOB SUMMARY
The Nurse Technician functions under direct supervision of a Registered Nurse as a vital member of the healthcare team. The individual will perform delegated functions of direct and indirect patient care under the supervision of a Registered Nurse and in accordance with organizational policies and procedures. The Nurse Technician will utilize skills achieved through their educational programs and/or orientation within the scope of the position.
