We are seeking a community-based Nurse Navigator to join our team in our team in Lehigh and Northampton counties and will be based at our Bethlehem, Pa location.  

 

 

POSITION OVERVIEW

The primary role of the Nurse Navigator is to function as a health advocate to aid individuals in receiving the physical health and behavioral health care they need.  The Nurse  Navigator will assist in coordinating medical and behavioral health appointments, hospital care and discharge to the community, arranging travel to appointments, and the development of personal plans for consumer health collaboration and wellness. Responsibilities include but are not limited to the following:

Provide integrated service coordination for an assigned group of consumers, including coordinating and monitoring behavioral and physical health care Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the consumer and any natural or professional supports they identify. Provide initial and on-going assessment of consumer's mental health, physical health, and substance use and response to treatment Review labs, physician visit summaries, physical and behavioral health data, medications lists, and ensure metabolic screenings are completed Support individuals in managing medication concerns/issues and monitor medication changes Provide Physical and Behavioral Health support and education to consumer about his/her health conditions. Consulate with behavioral health navigator to provide holistic care Engage with external and internal stakeholders and educate on the purpose of the Wellness Recovery Team. Collaborate and communicate with other healthcare staff involved in consumer's care to ensure consumers are receiving necessary care. Ongoing assessment of gaps in care and completion of referrals as needed.

 

Earn $33.81/hour.
