The CHNw Perinatal Navigation program will offer high touch, coordinated care for women at increased risk of preterm delivery or postpartum complications due to behavioral or socioeconomic risk factors, as well as comorbid conditions such as diabetes, hypertension, or a history of spontaneous preterm birth. Women who enter prenatal care early in pregnancy significantly reduce their risks and that of their unborn baby of preventable, adverse health conditions. Navigators will coordinate access to social service resources and sub-specialty resources and promote evidence-based care protocols to improve birth outcomes. Demonstrates performance consistent with professional standards of practice and care, the Nurse Practice Act, and the mission and goals of the organization. Manages patient care in the health care continuum to achieve optimum outcomes in a cost-effective manner. Demonstrates mastery of Registered Nurse role with skills in collaboration, delegation, negotiation, and management of resources. Exhibits motivation, commitment, and a sense of pride in professional nursing practice. The navigator will support the Anderson prenatal office/site, but will be required to cover other navigators on the team. The position can primarily work from home, but also expected to meet the patients in the office as needed.
Exceptional Skills and Qualifications
-Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.
-Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
-Five years or more of maternal & child health nursing experience required.
-Supporting prenatal care patients highly preferred.
-Previous medical office/outpatient experience preferred.
-Phone triage experience preferred.
-Excellent communication skills.

-Accountable and self-directed.
-EPIC experience a plus.
