Job Description​

Functions independently and accountable for utilizing the nursing process. Coordinates the care for a group of patients with the Multidisciplinary Team. Delegates tasks appropriately. Identifies hospital resources. Serves as a clinical resource for new employees. Demonstrates positive formal or informal leadership within unit or department.

In Virtual setting:

Functions independently and accountable for utilizing the nursing process. Demonstrates increasing discriminatory judgment in interpreting data and prioritizes appropriately. Initiates actions to improve nursing practice and patient care and unit processes. Serves as a day to day resource to colleagues at unit and department levels. Delegates tasks appropriately. Assumes preceptor responsibilities as needed. Utilizes appropriate hospital resources. Is a member of a collaborative care team who is responsible for providing tele-nursing care to a diverse population of patients across various departments within the SLHS. The following features govern the role: patient advocacy, prevention, telecommunication, proactive continuous monitoring, evidenced-based care, clinical decision support and humane, compassionate and ethical care. This will be an experienced nurse that monitors assigned patients using intelligent software.  They will be alerted to any changes in patient condition or trending that needs to be addressed. They will partner with bedside staff to address any concerns or changes in assigned patient’s condition. They will be located remotely at the Virtual care Center in Lees Summit. Goal is to identify early signs there might be an issue, address it, and reduce complications in this population.

Monitor intelligent software and vitals for changes and trends or patient condition.

Utilize good communication skills in tense situations.

Have years of experience to understand what they are looking for in trending for this population.

Partner with bedside team to monitor all assigned patients.

Provide early detection of changes or possible problems in this population.

Notify bedside team of possible issues and assisting them in activating resources needed to address issues.

Provide another set of eyes and ears monitoring this population with an additional piece of intelligent software.

Identify early signs to reduce complications.

In Community Hospital setting:

Demonstrate ability to mange care within the established standards of nursing care and practice as defined by the state.  

Appropriately triage patient and prioritizes needs, displaying a caring and responsive attitude.  

Initiate care to stabilize patient and assist in managing life support needs by collaborating with physician and clinical staff, demonstrating appropriate admission, discharge and transfer procedures.

Collect detailed patient history and symptoms, including vital signs, providing appropriate care in a timely and cost-effective manner.  

Collect laboratory samples and perform lab testing in accordance with CLIA regulations and established polices and procedures.  

Report adverse treatments or medications in accordance with current policies and procedures.  

Prepare patient meals.  

Participates in QAPI activities including participation in accreditation preparation and survey process, and maintenance of accreditation, state, and CMS standards.

In Hospital in Your Home setting:

Provides in-home nursing services and patient care to Hospital in Your Home patients. 

Completes assessment of patient’s physical and psychosocial status and needs.

Responsible for working with other members of the health care team when indicated to ensure optimal patient outcomes.

Executes orders and notifies physician and Command Center team of patient needs and changes in condition.

Demonstrates proficiency with clinical competencies and participates in-service training and continuing education opportunities. 

Provides education and counseling to patients, family members, and other caretakers to support patient’s care plan.

Documents patient visits, interventions, assessments, educational activities, and support provided.

Abides by legal and system rules, policies, and procedures.

All other duties as assigned.

In ASC setting:

Provide professional nursing care for ASC patients following established standards and practices.

Provide telephone triage, patient flow, and coordination of patient care.

Schedule urgent need patient appointments, identify learning needs for patients and families, provide advice and recommendations, insert intravenous access with medication administration and duties as assigned based on ASC need.

Delegate tasks to clinical staff in accordance with demonstrated competencies and consideration to scope of practice.

Serve as subject matter expert for patient education, assessment, interpretation, and documentation of patient care and testing.

Able to work in a team atmosphere as well as autonomously.

Provide excellent customer service skills and professional demeanor at all times.

May cover Medical Assistant, Medical Office Specialist, or other office duties as needed.
