This role is open to Oregon or Washington candidates. Job Summary The Registered Nurse/Office – Primary Care provides appropriate, compassionate remote support and guidance to patients or caregivers, utilizing good clinical judgement and approved protocols. The position is responsible for identifying the nature and urgency of a patient’s needs and determining the appropriate plan of care. Also, the position conducts patient outreach, education or advice and provide clinic support to the primary care team. Essential Responsibilities + Utilizes a broad range of general nursing knowledge and expertise to triage patient calls, faxes, or requests and develops plan of care using good clinical judgement and established protocols. + Makes outgoing calls to patients after a hospitalization to conduct a comprehensive assessment and develop plan of care based on their discharge needs. + Evaluates and assists with a patient’s medication needs, including refills and prior authorizations. + Provides disease specific teaching and counseling to patients and caregivers. + Communicates lab results to patient and provide relevant education based on their lab values. + Consults and collaborates with all members of the interdisciplinary team to develop and implement a comprehensive care plan based off assessment and individual patient care needs. + May use independent clinical judgement and decision making in applying protocols for managing patient care or advocating for clinical visits by team members. + Facilitates care that is based on the nursing process, evidence-based guidelines and is consistent with Oregon’s Nurse Practice Act. + Documents all patient-related communications in the electronic health record (EHR). + Collaborates with other members of the organization as appropriate to follow up on patient care needs, quality, and safety issues for the practice. + Develops and maintains professional boundaries with all patients/family/caregivers. + Assists primary care team with medication reconciliation and transitions in care support. Experience and/or Education Required + Associate degree or higher in Nursing + Current unencumbered RN license in Oregon + Minimum 2 years’ experience in nursing related to primary care or specialty clinic, geriatrics, home health or hospice Knowledge, Skills and Abilities Required Knowledge + Knowledge of Federal, State and Local regulations, including HIPAA privacy rules and regulations Skills and Abilities + Maintain patient/family and staff confidentiality in compliance with HIPAA laws + Participate in the coordination of the patient’s care in accordance with professional standards of practice + Practice within the scope of license, certification, or registration, where applicable, and as required by law and regulation. + Ability to take initiative and utilize innovative techniques and ingenuity + Excellent customer service skills, including the ability to interact professionally, patiently, and courteously over the phone and in person + Ability to adhere to organizational standards, policies and procedures + Ability to work both independently and collaboratively + Ability to react calmly and effectively in emergency situations + Ability to tolerate and manage stress and grief + Ability to interact with various departments throughout the organization + Ability to remain flexible and responsive when changes occur in patient activity, workload, and scheduling + Ability to plan, prioritize and coordinate multiple projects or tasks + Strong computer skills and a good working knowledge of Electronic Medical Record systems, MS office, Outlook, and the internet + Ability to work effectively with diverse individuals and groups + Ability to learn, focus, understand, and evaluate information and determine appropriate actions + Ability to accept direction and feedback, as well as tolerate and manage stress + Ability to see, read, hear, speak clearly, and perform repetitive finger and wrist movement for at least 6 hours/day Working Conditions Work Environment(s): ☒ Indoor/Office ☐ Community ☐ Facilities/Security ☐ Outdoor Exposure Member/Patient Facing: ☐ No ☒ Telephonic ☐ In Person Hazards: May include, but not limited to, physical and ergonomic hazards.
