Overview

The RN Supervisor is responsible for the quality of care provided for clients. Collaborates with physicians, family, caregivers, and support services to assure and improve client outcomes. Demonstrates professional management and leadership in clinical practice. Works with a high degree of independence to ensure optimal nursing, satisfaction outcomes are achieved in a timely and efficient manner. In collaboration with the Branch Manager, the RN Supervisor is accountable for overseeing the day-to-day nursing operations to ensure quality of care & services, regulatory compliance, support of business development & growth, achievement of key performance indicators, and quality outcomes. Responsible for staff training, employee skills validation, and direct supervision of caregivers (where applicable upon state regulations). Conduct initial home visits, supervisory visits, and coordinate/implement the client’s Plan of Care.

External Job Description Plans, coordinates, delivers, and oversees the delivery and outcomes of required healthcare related services/training programs on a routine basis in cooperation with Operations management; monitor both skilled and non-skilled healthcare services Directly supervises clinical personnel (where applicable by state regulation) Provides clinical teaching, competency validation at orientation and annually for Caregivers and nursing staff Ensures compliance with Company policies and procedures, state licensure and regulations, as well as state reimbursement regulations Leads case conferences and other client-related meetings Chairs or attends committees as appropriate, safety, infection control, and critical incident review Facilitates communication and serving as a liaison between and among nursing, caregivers, family and/or guardians, community medical providers and professional consultants Directly oversees initial start of healthcare, assessment, and first home visit with ongoing performance monitoring to identify/reduce risks and improve client outcomes Directly supervises the implementation, coordination, maintenance, evaluation, and termination of care plan/services, medical records, and physician orders Markets to healthcare providers in service area as requestedProvides skilled nursing care/treatment to clients per Physicians Orders and Plan of CareAccurately documents nursing actions of all care given, individual’s health status, and communicate with family and physiciansDocuments the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needsProvides skilled nursing treatments per physician’s orders and within the nurse’s scope of practiceAdministers and manages medication as neededProvides health teaching of client and assisting client in learning appropriate self-care techniques and safety measures Conducts nursing assessment visit as neededReports all incidents per operation procedure and regulatory standardsFollows policies and procedures according to operation, state and regulatory standardsUses of safe body mechanics to prevent personal injury to self or clientMaintains confidentiality of patient related information and honor patient rightsMay be asked to cover after hours, weekends, and holiday on call to meet the needs of clientsOther duties as assigned Qualifications Bachelor’s Degree in Nursing, Social Services, or related healthcare field of studyHealthcare management experience with multiple age groupsMinimum of at least two years of clinical experience, and at least one year in related management/supervisory role in Home Care and/or Home Health Meet all pre-employment requirements per company and/or state regulations Current and unrestricted registration and license in the state of the operationCPR professional certificationMaintain state regulatory certifications, as required by state/program requirements Functions within the Nursing Scope of Practice and Nurse Practice Act (NPA)Professional appearance and demeanorGeneral knowledge of computers and the ability to learn industry specific software and documentation applicationsExcellent oral, written, and interpersonal communication skills with all levels of personnel, internal and external to the companyExcellent organizational and time management skills with a keen attention to detailsCapable of working responsibly with highly confidential information Ability to work effectively independently or in a team environmentAbility to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issuesAbility to learn and complete various educational and position specific goals About our Line of Business
