Summary
Identifies patients for referral to a multidisciplinary team or appropriate provider using standardized screening tools, within the EMR. Informs patients of information needed for initial visit: medical records, test results, release of information, etc. Ensures those items are present and/or completed for initial visit. Ensures patient receives applicable information prior to initial visit. Conducts initial standardized screening to identify any barriers to care and educational needs. Repeats screening at regular intervals or as needed. Provides timely referrals and facilitates scheduling of appointments to appropriate support services based on screening results and as needed. Serves as a primary point of contact for patients, family members, and referring physicians to access care. Decreases fragmentation of care and coordinates services. Coordinates diagnostics, procedures, and specialty appointments, as necessary. Serves as a clinical liaison for the new and existing patients and is responsible for managing their care in collaboration with a team of healthcare professionals to ensure timely, high-quality care across the patient’s care continuum. Assists in administrative functions and in the development and implementation of patient care protocols, clinical pathways, and in updating patient policies and procedures. Coordinates efforts for treatment and promotes quality improvement and educational efforts. Opportunity for professional development.

Job Duties

Helps patients and their family members "navigate" their medical care by helping them find solutions to treatment-related problems.Contributes to the health care team's development of the patient's plan of care. Documents all aspects of care coordination. Facilitates prevention, detection, and screening activities as appropriate.Assesses and monitors the continuity of care and expedites and coordinates the delivery of services to facilitate a patient's progress through their care.Effectively screens patients for a variety of complex Vascular conditions via EMR (EPIC). Using the EMR (EPIC and other systems) to identify patients who may benefit from the Limp Preservation Program and/or care for Peripheral Artery Disease.Collaborates with the patient's medical team for follow-up and monitors patients' progress.Participates in the development, process implementation, and review of quality metrics.Participates in the development and implementation of patient education materials as well as community outreach activities.Identifies appropriate indicators to monitor and track patients in conjunction with marketing, patient data advertising campaigns, and patient referral outcomes.Compiles data, tracks outcomes, and makes recommendations for process improvement.Assists in the identification and reporting of quality issues to improve efficiency. Engages in quality assessment, utilization review, risk management, and infection control.Functions as a clinical liaison for new and existing patients using approved criteria and direct clinical experience.Provides referrals and support for follow-up care for patients who are uninsured or underinsured.Performs other duties as assigned.

Minimum Qualifications

Specialized Diploma Nursing2 years Experience working as an RN.Ability to maintain confidentiality of records and information.Knowledge of patient education techniques and principles.Knowledge of community resources and how to access them effectively and efficiently.Must demonstrate skills and knowledge in obesity management theories, practices, and techniques.Proficient in Microsoft Office applications.Skilled in both medical and psycho-social assessment.RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Bachelor’s Degree Nursing


Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
