 Registered Nurse (RN) – Physician Office - Rheumatology Center - Richmond, VA

Summary of Primary Function
Delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patient’s problems/needs, and evaluates the process and outcome of nursing care; coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members.

  

Essential Job Functions

In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness; administers medications in a safe manner consistent with the State of Practice and organization policies and procedures; serves as point of contact for patients' care coordination throughout hospital departments.  Acts as a patient safety advocate by participating in ongoing quality improvement in the department.  Incorporates the professional practice model into care delivery.Demonstrates skills and judgment necessary to implement medical plan of care, nursing interventions, and assist with procedures as necessary for patient care.  Evaluates the identified problems, care provided and ensures patient’s responses are reassessed to meet patient needs to ensure care delivery.Develops, discusses, and communicates a realistic plan of care in collaboration with each provider and/or patient in order to address all identified needs.Maintains up to date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team.  Partners with physicians to ensure physician orders and directives are effectively implemented.Within the scope of the position and RN licensure, answers staff questions and educates medical assistants, directing work when appropriate, to ensure excellent customer service and patient satisfaction.Promotes effective working relations/works effectively as a office team inter- and interdepartmentally to facilitate the office’s ability to achieve objectives.Monitors electronic medical record for deficiencies and delinquencies and notifies appropriate provider(s) for reconciliation.Demonstrates respect/regard for the dignity of patients, families, visitors, and associates to ensure a professional, responsible, and courteous environment.
