Summary of Primary Function/General Purpose of Position
The RN Inpatient Rehabilitation functions as a care provider of a multidisciplinary care team responsible for the delivery of patient care through using the nursing process of assessment, diagnosing, planning, implementation, and evaluation. The RN possesses the specialized knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness. The RN possesses the basic knowledge of a patient's age and cultural diversity into the provision of patient care, demonstrates strong skills related to effective time management and prioritizing patient care, contributes to the provision of quality nursing care through performance improvement initiatives, collaborates with other professional disciplines, and supports the mission of the ministry to provide physical and psychological support to patients and families.
Essential Job Functions
• Collaborates with members of interdisciplinary teams to establish goals and strategies for meeting the discharge or continuing care needs with the patient.
• Performs and documents complete patient assessments.
• Initiates and maintains current nursing care plans for all assigned patients.
• Identifies patient learning needs to provide individualized and comprehensive teaching.
• Reinforces the teaching done by other members of the rehabilitation team and other healthcare disciplines and provides resource materials for patients and families.
• Fosters independence in patients and families for self-care skills, self-management, decision making, and goal development
• Administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures.
• Serves as a point of contact for patients' care coordination throughout the hospital departments.
• Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
• Serves as a patient advocate in collaboration with spiritual care, palliative care, and ethics.
• BSMH nurses are expected to practice and deliver care in accordance with the core concepts and intention of the BSMH Nursing Professional Practice Model.

Licensing/Certification
Current RN license in state they are working or covered by compact (required)
BLS Basic Life Support- American Heart Association (preferred upon hire, must obtain from approved source prior to direct patient care)
Certified Rehabilitation Registered Nurse (CRRN) (preferred)
Medical-Surgical Nursing Certification RN-BC (preferred)
Certified Medical-Surgical Registered Nurse (CMSRN) (preferred)
Certified Neurological Registered Nurse (CNRN) (preferred)
National Institute of Health (NIH) Stroke Certification (preferred)
Gerontological Nursing - ANCC (preferred)
Education
Bachelors of Science Nursing (preferred)
Work Experience
1 year of clinical patient care experience (e.g. Patient Care Tech (PCT), Licensed Practical Nurse (LPN), Certified Nurses Aide (CNA)) in an acute care setting, long-term care facility, or physician practice (preferred)
1 year of experience in an acute rehabilitation facility (preferred)
