Starting Pay - $34.33 - $42.91 (Based on Experience)

Completion of an accredited professional nursing program Current Minnesota Registered Nurse License BLS certification within 90 days of hire One to three years nursing experience preferred Previous anti-coagulant experience preferredJob Responsibilities:
Practice of Professional Nursing includes but is not limited to:
Required to work to the full scope of RN Licensure. Collaborates with the healthcare providers to develop and coordinate an integrated plan of care for anticoagulation management therapy. Primary point of contact for anticoagulation management therapy. Primary educator for patients and family members for anticoagulation management therapy. Assumes primary management of anticoagulation management therapy to include: Dose adjustments requiring strong critical thinking skills and knowledge to include assessment of patient medications, diet, and health changes. Ordering of medication refills and lab testing. Initial and continued education relating to high-risk medications to patients and families; to include in-person or telehealth visits. Provide advice on adverse reactions and next step instructions. Strong knowledge base to recognize medication, diet, and health interactions that can affect a patient’s well-being. Provide dosing recommendations to providers for surgical procedures, dental procedures, or a patient with no maintenance plan dosing. Provide coordination of care with providers based on received referrals. Provide pre and post procedure recommendations to include blood clot and bleeding risk assessments. Perform extensive reviews of patient medical records. Participates in the development of healthcare policies, procedures, and protocols. Accountable for the quality of care delivered, recognizing the limits of knowledge and experience; addresses situations beyond the nurse’s competency; and performs at the level of education, knowledge, and skill ordinarily expected if an individual has completed an approved professional nursing education program. Maintains OMC organization competencies and nursing competencies pertinent to area of practice. Manages information in accordance with state and federal regulations. Assists in proper training of new staff. Attend departmental meetings as the experts in anticoagulation management therapy. Assist in shadow-based training for pharmacy students and current providers. Coordinate transfer of patient care services. Assist in management of supplies based on office needs. Performs other duties as assigned.
