abnormalities of the heart and great arteries including the heart muscle heart valves and supporting structures Procedures include TAVR TMVR MitraClip and LAA implants The Structural Heart Nurse is a valued member of this provider led team serving as a liaison to our patientsUnder provider direction the RN provides skilled nursing care to patients in a variety of clinical settings Scope of responsibility is characterized by use of nursing process to assess plan intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices standards protocols and guidelines This position reports to a Practice ManagerHelps to facilitate appointments for consults clinic and support services both inpatient and outpatientCommunicates and assists in coordinating patient care with leads of cardiology team in order to ensure proper transition of careEnsures that data is completed appropriately for registriesProvides office based nursing care in collaboration with providerAssesses physical and psychosocial needs and environmental factors affecting patients and their families formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and interventionCommunicates with provider regarding patient needs nursing assessments and recommendationsTakes direction and initiates actions crossadditional training that will allow seamless patient careCollects and documents current health status and medical history and assists in scheduling patient office visitsReviews patient records including preoperative records to ensure completeness and reviews outstanding issues with providerDemonstrates independent nursing actions based on assessment and problem identificationUnder direction of provider calls in prescription medications to pharmacies and observes and documents patientrsquos response to pertinent medications Documents accurate and ongoing assessment of patientrsquos needs problems capabilities limitations progress toward goals and response to careTriages telephone calls and displays ability to initiate appropriate nursing intervention in prompt precise and professional
