Registered Nurse RN Case Management Per Diem
**Employment Type:** Part time **Shift:** Day Shift **Description:** At **Johnson Memorial** **Hospital** the **Case Management Department** we provide quality care for patients by organizing patient care and treatments in incorporating all members of the care team. Our professionalism, punctuality, attendance, teamwork, and empathy are at the core of how we represent Johnson Memorial Hospital care and care for our patients, together, we positively impact the lives of all our patients and their families. **What you will do** In this critical role you will assist in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient. **Minimum Qualifications** + **Education** : Graduate of an accredited college or university affiliated nursing program. BSN graduate preferred. + **Licensure** : Current Licensure as a Registered Nurse RN in Connecticut. + **Certification** : BLS (AHA/ARC). Certification in Case Management (ACM, CCM) required within one year of employment after meeting minimum requirements. **Preferred Skills:** + **Experience:** three years of diversified, progressive experience with at least 1 year of recent experience in an acute care facility or public health nurse setting. + Including an in-depth understanding of and skill in using many nationally recognized medical utilization criteria sets (i.e. MCG, InterQual, and others as needed). + Requires a solid understanding of managed care methodologies, HCFA/OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities of the Continuum of Care **Work Schedule** : Per Diem– Rotating Schedule- Days **Ministry/Facility Information** **Johnson Memorial Hospital** and Home & Community Health Services provide a continuum of health care services to those living and working in north central Connecticut and western Massachusetts.
