Description:This position is for a RN Case Manager at Trinity Health Hospital in Grand Rapids, MI. Trinity Health Grand Rapids is a Magnet Designated Hospital! What you will need:

Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire; ADN only, if nationally certified in Case Management.Current RN license in the State of Michigan.Active BLS CertificationPreferred: Certification in Case Management or other related professional certification.Two to five years of medical/surgery or chronic disease management experience in an acute care settingPreferred: Previous case management experience, experience as a participant in continuous quality improvement, completion of self-management support training.
