Community Care Coordinator RN Position Summary

The Community Care Coordinator RN serves in an expanded nursing role to collaborate with TidalHealth practices and other Primary Care Providers as well as TidalHealth partners as necessary. The Community Care Coordinator RN ensures safe transitions of care that will prevent readmissions and support best outcomes for patients and their families. The Community Care Coordinator RN also ensures the delivery of quality, efficient, and cost-effective healthcare services and assesses, plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the client's health status. The Community Care Coordinator RN works with high-risk populations in collaboration with providers and the healthcare team and integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare. These primary care offices can span from Pocomoke, MD to Millsboro, DE.

Community Care Coordinator RN Position Requirements

Three to five years’ experience in clinical nursing required. Case Management experience required. Knowledge of the basic concepts and principles of care coordination and managed care required. Must have valid driver’s license, a driving record deemed appropriate by TidalHealth, and reliable transportation. Three to five years’ experience in clinical nursing required. Community Care Coordinator RN Work Schedule

This is a salaried, exempt position. Could be expected to attend occasional evening meetings Remote work available 1 day per week
