SUMMARY
The Hospice Liaison Registered Nurse is responsible to facilitate the patient discharge from the facility setting to hospice care services in the appropriate setting and level of care. The Hospice Evaluator Registered Nurse will provide superior customer service by modeling the Brand Promise and Core Values.

LOCATION: Unity Hospital/Rochester General Hospital
DEPARTMENT: Home Care Coordination and Intake
SCHEDULE: Weekends, Fri 12-6pm, Sat & Sun 8-8pm.
ATTRIBUTES
Valid NYS License as an RN required
Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
Experience in working in partnership with the interdisciplinary healthcare team.
Experience utilizing an Electronic Medical Record and Microsoft office
Demonstrate ability to apply decisive, sound, objective clinical judgment for cases with varying complexity
RESPONSIBILITIES
Evaluation & Collaboration. Evaluates eligibility, needs and care resources for each patient referred to RRH Hospice Care. Obtains insurance information and authorizations for appropriate reimbursement. Coordinates all services including but not limited to professional, paraprofessional services, durable medical supplies, equipment, etc., as the care plan dictates. Collaborates with facility care coordinators, social workers, and physicians to find solutions for challenging patients requiring hospice care as a part of the facility discharge plan.
Assessment & Development. Completes a comprehensive clinical assessment for hospice services and develops a hospice plan of care with the patient/caregivers to ensure a safe plan. Obtains MD order to support hospice plan.
Documentation. Complete all required documentation in patient’s facility record; gather and analyze data and complete all assigned reports. Completes all required documentation in patient’s facility record and RRH Hospice Care required documentation per requirements.
