Overview The Registered Nurse is responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The RN is responsible for performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act. The RN coordinates care with members of the health care team and delegates responsibilities to others as appropriate Responsibilities Job Description Assesses patient needs and identifies expected nursing outcomes. · Involves the patient, family and other health team members in gathering data. · Uses an ongoing, systematic process to gather relevant information (biological, psychological, spiritual and social aspects) from patient and family. · Analyzes data and information to identify potential risks, prioritize needs and determine priority problems. · Identifies outcomes that are age appropriate, culturally appropriate, realistic and attainable. · Validates these with patient, family and other health team members. · Documents data and identified outcomes. Develops an individualized nursing plan of care. · Collaborates with patient, family and other health team members to develop a nursing plan as part of an integrated, comprehensive plan of care · Develops an individualized plan based on age, social and cultural considerations, condition and other relevant needs. · Determines nursing interventions based on identified needs. · Identifies interdisciplinary resources needed to assist in implementing the nursing plan of care. · Prioritizes interventions based on patient acuity, available resources, and patient/family preferences and needs. · Documents and updates the plan of care to assure continuity throughout the continuum of care Implements nursing interventions based on the plan of care. · Provides care according to the plan, and in accordance with all relevant policies, procedures and nursing practice guidelines. · Coordinates nursing care with the care of all other health team members. · Initiates age and culturally appropriate measures to support and meet patient needs while achieving expected outcomes. · Initiates patient/family teaching as appropriate to age, readiness to learn and available resources. · Collaborates, consults, or makes referrals if unable to perform safely and independently. · Delegates direct and indirect patient care activities to appropriate personnel based on needs, condition, potential for risk/harm, complexity of the skill and predictability of the outcome. · Assures all actions are based on scientific, ethical or ecological principles; proven effectiveness; past experience; practice standards; patient/family requests and values; or, legal parameters. · Uses the full range of therapeutic modalities to support and augment the healing process, moving from least intensive and invasive to more, depending on need and context. · Documents interventions according to practice guidelines. · Anticipates, recognizes and responds to emergent situations. Evaluates care and patient response to interventions and expected outcomes. · Systematically and continuously evaluates patient’s response to interventions and the effectiveness of the plan of care · Modifies the interventions based on patient response and changing patient condition. · Involves the patient, family and other health team members in evaluating care. · Uses ongoing assessment data to revise patient needs; priority problems, nursing outcomes and the plan of care. Uses communication strategies to achieve desirable outcomes. · Communicates verbal and written information in a clear, concise, accurate, and timely manner. · Uses communication strategies appropriate to patient/family needs for information, education, and support. · Collaborates and communicates effectively and respectfully with the patient, family and members of the health care team within and across care settings to promote partnership-based health care Demonstrates the ability to advance care by utilizing standards, guidelines, care pathways and continuous improvement to enhance patient outcomes. · Identifies gaps in care and practice and brings them forward to applicable person(s) · Utilizes improvement methodologies · Incorporates data and information into practice to continuously improve or innovate care Promotes a culture of evidence based practice (EBP) by participation in any of, but not limited to, the following activities: · Utilizes evidence based policies in clinical practice · Attends and participate in learning opportunities about EBP · Member of hospital or area based nursing practice council or research council · Completes an evidence based practice project (e.g. policy development or poster presentation) · Presents an EBP finding to your peers · Journal club member · Critiques and circulate a journal article relating to nursing practice · Reviews the literature on a practice question · Develops a standard/pathway/guideline for patient care · Participates in a research study · Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served. Information Technology: • Basic Computer Competencies – using the computer, email, basic use of word / excel, and web browsing • Information Literacy – ability to determine information needed, efficiently access and critically evaluates that information, apply use information for patient care, and evaluate the use of information. • Information Management – verbalize importance of health information systems for nursing practice, maintain confidentiality, privacy and security of protected patient health information, adequately navigate health information systems, and abide by policies and procedures pertinent to health information system use. Qualifications Current MN RN License BLS for healthcare providers ACLS acquire within 2 months of hire Must be able to respond to the hospital within 30 minutes of receiving a page Minimum of two years IR, Critical Care Experience (experience with hemodynamic monitoring, advanced airways, vents, critical care drips, iv pressers, and ACLS related drugs) in one or more of the following areas: ICU, ED, PACU Previous IR/Interventional Radiology or Cardiac Cath Lab experience EEO Statement EEO/AA Employer/Vet/Disabled: All qualified applicants will receive consideration without regard to any lawfully protected status Benefit Overview Although this is considered a non-benefit eligible position, Fairview offers employees the option to participate in the Fairview 403 (b) plan, earn Sick and Safe Time and early access to earned wages. Compensation Disclaimer The posted compensation range corresponds to the minimum and maximum pay rates outlined in your union agreement for the respective job classifications at the time of job posting. An individual's pay rate within this range may be influenced by various factors, including FTE, skills, knowledge, educational background, and qualifying experience as specified in the contract.
