Job Summary:
The RN Navigator is responsible for Admissions; Discharges; RN Navigator Referrals; collaboration with Case Managers and LSW for Discharge Planning, Service Recovery and adjunct to primary RN's Patient Education. This includes identifying the high-acuity/ high risk for readmission patient populations, and promoting self-management goals to achieve individualized behavioral and treatment goals that optimize health outcomes. The RN Navigator will work with the acute care team to develop a plan of care that best serves the needs of the individual patient. The RN Navigator will be responsible for documentation, support data collection, analysis, and presentation to show results of the program and is also responsible for working with Clinical Quality and Medical Leadership to continuously improve quality performance and clinical outcomes.
Skill Qualifications:
Experience with clinical pathways, data analysis, and health care operations. Strong skills in independent problem solving and process management .Strong interpersonal skills and ability to work collaborative with patients, non-clinical staff, clinical staff and Physicians.
Education & License Qualification:
Licensed as a registered nurse in the state of employment with 3-5 years clinical experience. 2 year/Associate Degree required – Bachelors Degree required within 3 years of employment.
