GENERAL RESPONSIBILITIES \n
Under the supervision and leadership of the Nursing leadership team, is responsible for providing patient care either in person or virtually as well as coordinating day-to-day nursing care and functions provided by nursing care team members in accordance with established standards of patient care and all Terry Reilly policies and procedures. Ensures support for clinicians, fosters teamwork and quality patient care, and implements and enforces protocols, policies and procedures in accordance with a Patient Centered Medical Home. Takes lead role in responding to crisis situations\/critical patients.\n
ESSENTIAL DUTIES\n\nIn accordance with Idaho State Registered Nurse Practice Act, assesses patient needs, provides education services, and assists the clinical staff in the conduct of a comprehensive Patient Centered Medical Home care delivery system. Promotes an efficient, positive, and safe environment for patients and care team members.\nProvides direct patient care both in person and\/or virtually. Performs nursing procedures, provides appropriate patient education, generates all necessary EHR documentation and completes support duties in a timely, professional, and efficient manner.\nProvides assessment and care to walk-in patients and arranges for them to receive prompt care, as per established protocols. \nCompletes RN Screening, RN Refill, and any other standing orders as available.\nAdditional clinical tasks\n\n
MINIMUM QUALIFICATIONS\n\nCurrent Idaho R.N. or L.P.N license\nCurrent BLS certification.\n1-year direct care nursing experience.\nStrong problem solving, time management and computer skills.\nAbility to communicate effectively and in a professional manner with all care team members, supervisors, and patients. \nAbility to establish and maintain a positive, efficient, and safe working environment.\n\n
PREFERRED QUALIFICATIONS\n\n2+ years' experience in a variety of physical and\/or virtual care environments including but not limited to: inpatient, family practice, ED.\nBachelor's degree in Nursing.\nExperience in a community health clinic environment working with a wide variety of patient populations, including patients living in poverty and individuals of minority groups. \nBilingual abilities (English\/Spanish).\n\n
