UPMC is searching for a full-time experienced nurse for our Rehab Coordinator role! This salaried position will work out of UPMC Presbyterian Hospital (200 Lothrop Street, Pittsburgh, PA 15213) with Monday-Friday daylight hours and occasional rotating Saturdays (approximately 4 per year). No nights or holidays are required!
In this role, you'll have the autonomy to evaluate patient charts and determine readiness for the transition to our UPMC inpatient rehab locations. There is no direct patient care in this position, and you can look the part while wearing business casual dress code! Experience in discharge planning and/or case management as well as high-paced acute hospital or rehab settings is preferred. A bachelor's degree in nursing is required as well as at least 4 years of previous experience in a rehab, skilled nursing or acute care setting.
The Rehab Coordinator assesses patients to evaluate for medical appropriateness to an inpatient rehabilitation facility. The Rehab Coordinator works with the multi-disciplinary team in resource management, discharge planning, and care facilitation to coordinate the admission of appropriate patients into a UPMC inpatient rehab facility. Promotes the achievement of facility census requirements through direct contacts and appropriate referral sources.
**Responsibilities:**
+ Provide education about services, facilities and resources of UPMC Rehabilitation Institute/skilled nursing facilities for patients and their families; physicians, case managers, social workers, PNCC's and others.
+ Solicit feedback from UPMC rehabilitation and skilled nursing management, case management leadership and social work service regarding rehabilitation resource and skilled nursing needs and make recommendations for service improvement.
+ Review patient records and obtain any relevant patient information, assessing for patient appropriateness and medical stability to facilitate a timely admission and transfer to an appropriate UPMC IPR/skilled nursing facility.
+ Conduct on site assessments and identify potential rehabilitation/skilled nursing candidates for appropriateness and medical stability for admission to the inpatient rehabilitation and skilled nursing programs within UPMC.
+ Work in collaboration with PM&R department as necessary to obtain necessary documentation to support inpatient rehab admission, to facilitate discharge recommendations and to augment peer to peer review.
+ Maintain accurate daily logs of patients referred and complete monthly status report within designated time frame.Attend meetings and designated training sessions in order to maintain current knowledge of all payer and regulatory requirements, UPMC Rehabilitation Institute/skilled nursing facility policies and procedures, community resources. Ensures compliance with all payer and government regulations. Serves as a role model of excellent customer serviceInitiate and participate in performance improvement initiatives.
+ Assist in pre admission care planning. Identifying and recommending special equipment needs and precautions to assure safe continuum of care and making recommendations regarding cost effective and appropriate admission orders.
+ Complete documentation and paperwork related to patient admissions accurately and within acceptable time frame.
+ Establish and maintain UPMC rehabilitation/skilled nursing resource and education stations/locations. Place and distribute brochures, posters, and other materials as needed. Distribute UPMC promotional items for marketing to internal and external referral sources.
+ Travel daily to various healthcare entities.
+ Make sales and census development contacts while in the hospital and with other referral sources and achieve associated admission goals.
+ Work with UPMC staff to facilitate a seamless transition for patients and families to inpatient acute rehabilitation. Addresses complex clinical and social situations efficiently in order to avoid unnecessary delays in discharge from acute care to inpatient rehab or other post-acute setting. Proactively identifies barriers to discharge and works with care management, PMR and multi-disciplinary team to facilitate discharge.
+ Develop and maintain knowledge of the inpatient rehabilitation/skilled nursing programs and services within UPMC.
+ Coordinate and communicate patient information via telephone, fax, electronic means or other mechanisms to an appropriate designated intake person at accepting facility while maintaining confidentiality and privacy in compliance with HIPPA guidelines.
+ Graduate of accredited school of professional nursing BSN required or UPMC approved certification in Rehab
+ 4 years previous experience in a physical rehabilitation setting, skilled nursing setting or other equivalent and/or acute care experience required
+ Experience in Marketing preferred.
+ Strong computer skills and the ability to work independently, multi-task and problem-solve.
+ Knowledge of various health insurance policies.
+ Professional oral and written communication skills.
+ Ability to establish and maintain effective working relationships with a variety of internal and external referral sources.
+ Ability to market the UPMC rehabilitation services and programs to interested internal and external referral sources. **Licensure, Certifications, and Clearances:** Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
