Description
POSITION:Day Habilitation Trainer / Transportation Aide/Relief Day Habilitation Worker
DEPARTMENT: Day Habilitation
DIRECT SUPERVISOR: Day Habilitation Specialist

DUTIES AND RESPONSIBILITIES:The Day Habilitation Trainer/Transportation Aide will assist the Day Habilitation Specialist in their assigned core room or discipline with the implementation of individual treatment plans, participant supervision during non goal planned periods, and ongoing development of a habilitative program environment. The Day Habilitation Trainer/Transportation Aide is also responsible to assist in the safe transportation of passengers in transit to and from program site. The duties and responsibilities include, but are not limited to:
1. Actively cooperate with the Day Habilitation Director and other program staff in order to achieve the goals and objectives of the Day Habilitation Program.
2. Responsible for implementation of goal plans and data collection to monitor progress. Goal Plans are to be based upon the person’s valued outcome as identified in their ISP.
3. Responsible for transporting individuals to volunteer and recreational activities in the Community. Also provide the necessary supervision, training and support during these outings.
4. Responsible for development of individual goal plans in accordance with the 3IP and 5 governing principles as assigned by the Direct Supervisor or Program Director.
5. Responsible for maintaining accurate daily participant records, charts, logs, and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies.
6. Responsible for maintaining an orderly, clean, safe, and habilitative core room environment at all times, and addressing and/or reporting environmental deficiencies to the Direct Supervisor as they are perceived or reported.
7. Responsible for program-wide responsibilities as assigned by the Direct Supervisor. These responsibilities could include, but are not limited to:
a. Ordering Supplies for core room with approval from the Direct Supervisor.
b. Lunch and/or Transportation duty.
c. Assisting with participant transportation.
d. Monitoring janitorial service and informing Direct Supervisor if any concerns.
e. Participating on program or agency committees.
f. Assisting with program projects, i.e., staff training, research, etc.
g. Trainers will be responsible for covering other core rooms during staff shortages.
h. Trainers will be responsible for changing participants & assisting in individual toileting if need be.
I. Provide assistance during behavioral emergencies which include, but is not limited to counseling, verbal calming, supervision and/or physical intervention.
8. Attend and successfully complete all training as required by OMPDD regulations and agency policy.
9. Responsible for complying with all waiver Regulations which apply to Day Habilitation Programming, Part 624 and Part 633 Regulations in total and reporting violations, deficiencies and/or incidents to the Direct Supervisor as they are discovered or indicated.
10. Active consultation/communication with Direct Supervisor and professional staff to insure individual's progress/safety and regulatory compliance.
11. Responsible for modeling appropriate behavior, appearance, interpersonal relationships, and communication whenever in the presence or probable presence of program participants.
12. Responsible for addressing and/or reporting all observed or suspected situations which adversely impact on the health and safety of any/all program participants.
13. Responsible for communicating with outside agencies, family, and residential programs to assure consistency in treatment. .
14. Responsible for completing monthly progress notes, semi-annual and annual reviews as assigned by or in the absence of the Direct Supervisor.
15. Assist in boarding vehicles and disembarking from vehicles.
16. Assist driver in maintaining cleanliness of vehicles.
17. Monitor passengers and maintain orderliness on vehicle to ensure passenger safety and adherence to safety regulations.
18. Complete all mandatory training in accordance with OMPDD and agency policy which includes, but is not limited to First Aid, CPR, SCIP, Medication Administration, 3 IP’s, 5 Governing Principals, Active Treatment, etc.
19. Responsible for planning activities which will foster community integration.
20. Any other duties as directed by the Transportation Coordinator.
21. Perform other duties and responsibilities as assigned by the Direct Supervisor.
Requirements
SPECIAL REQUIREMENTS
+ Candidate must be able to lift 50 pounds and not have any lifting restrictions which would prohibit him/her from assisting with toileting, transfers, physical intervention, lifting, etc.
+ Possess a valid driver’s license and good driving record acceptable to the agency’s insurance carrier.
+ Maintain certification in First Aide, CPR, SCIP and Medication Administration.
+ Responsible for following agency safety and health policies, procedures and rules.
QUALIFICATIONS
A two year Associates Degree in Human Service Field & experience working with the developmentally disabled.
Or
A high school diploma and one years experience working with developmentally disabled.
Or
A combination of education and experience that meets the needs of the reviewing agencies.
Salary Description
$16.00 per/hr
