Job Details · Location: Remote, but candidates should live near Nashville, TN. · Start Date: ASAP · Full Time – No Weekends · Negotiable Pay/ Weekly Compensation · Duration/Schedule: 3 months with likely extensions · Patient Volume: Complete 3+ patient visits daily Responsibilities · Maintain quality of care standards · Assist patients with enrolling in educational programs · Knowledge of disease diagnosis and prevention · Assess and evaluate patients' health status · Implement and follow up on care plans · Document patient information comprehensively and timely · Counsel and educate patients and families about health benefits and programs Requirements Experience: 2 years of direct patient care relating to polychronic conditions Polychronic conditions, also known as multiple chronic conditions (MCCs), refer to the presence of three or more chronic diseases in a single patient.
